腹主动脉的应用解剖学研究

【摘要】: 目的通过收集成人尸体标本和螺旋增强CT影像学腹主动脉资料对肾动脉下腹主动脉至双侧股动脉进行应用解剖学研究,了解肾动脉下腹主动脉、双侧髂总动脉、股动脉的长度和直径的解剖学资料,以及肾动脉下腹主动脉至股动脉分叉部长度与身高的关系,为补充腹主动脉体质资料和指导腹主动脉疾病的治疗提供依据。 方法采用50具健康成人尸体标本(男性33例,女性17例),,测量身高,解剖左右肾动脉、肾动脉下腹主动脉、左右髂总动脉及股动脉分叉部,分别测量肾动脉下腹主动脉至左右股动脉分叉部的长度,测量左右髂总动脉长度,测量肾动脉平面腹主动脉的直径,比较左右肾动脉位置的高低。对215例接受腹部增强螺旋CT的非血管疾病病人直接在电脑上通过标尺测量肾动脉平面的腹主动脉的前后径及左右径,测量左右髂总动脉在分为髂内外动脉上方的直径。将所测数据用SPSS11.5统计分析软件进行相应的统计学分析。 结果肾动脉下腹主动脉的长度为90.1±8.32mm,。肾动脉下腹主动脉的直径为17.89±3.25mm,不同性别成人的身高与肾动脉下腹主动脉长度有直线回归和较强的相关关系,回归方程为:成人男性的肾动脉下腹主动脉至分叉部长度=0.064×身高—1.159厘米,成人女性的肾动脉下腹主动脉至分叉部长度=0.084×身高—3.734厘米,回归系数的T检验和方差分析等价(P<0.05),相关系数r均大于0.6。同样的身高也与肾动脉下腹主动脉至双侧股动脉分叉部的长度有直线回归和较强的相关关系,回归方程为:成人男性的肾动脉下腹主动脉至左股动脉分叉部长度=0.192×身高—2.706厘米,成人男性的肾动脉下腹主动脉至右股动脉分叉部长度=0.200×身高—3.581厘米,成人女性的肾动脉下腹主动脉至左股动脉分叉部长度=0.224×身高—7.414厘米,成人女性的肾动脉下腹主动脉至右股动脉分叉部长度=0.314×身高—20.9厘米,回归系数的T检验和方差分析等价(P<0.05),相关系数r均大于0.6。而身高与腹主动脉直径无直线回归和相关关系(P>0.05)。 根据影像学资料得到的数据分析得出:肾动脉下腹主动脉的前后径为15.53±2.39mm,左右径为15.03±2.09mm;左髂总动脉终端的直径为8.47±1.05mm,右髂总动脉终端的直径为8.59±1.17 mm。 结论准确了解肾动脉下腹主动脉的应用解剖学资料丰富了国内腹主动脉的体质解剖资料,同时对破裂腹主动脉瘤的急诊腔内修复治疗及开腹手术有重要指导意义。
[Abstract]:Objective to study the anatomy of abdominal aorta from inferior renal artery to bilateral femoral artery by collecting adult cadavers and spiral enhanced CT images of abdominal aorta. The length and diameter of bilateral common iliac artery and femoral artery, and the relationship between the length of the inferior abdominal aorta to the femoral artery and the height of the bilateral common iliac artery and femoral artery. To supplement the data of abdominal aorta constitution and guide the treatment of abdominal aortic disease. Methods Fifty healthy adult cadavers (33 males and 17 females) were measured their height, left and right renal arteries, inferior abdominal aorta, left and right common iliac artery and femoral artery bifurcation were dissected. The length of the inferior abdominal aorta to the bifurcation of the left and right femoral arteries, the length of the left and right common iliac arteries and the diameter of the planar abdominal aorta of the renal artery were measured. To compare the position of left and right renal arteries, the anteroposterior and left diameter of abdominal aorta in renal artery plane was measured directly on the computer in 215 patients with non-vascular diseases undergoing abdominal enhanced spiral CT. The diameter of left and right common iliac arteries was measured above the internal and external iliac arteries. The measured data were statistically analyzed by SPSS11.5 software. Results the length of inferior abdominal aorta of renal artery was 90.1 卤8.32 mm and the diameter of inferior abdominal aorta of renal artery was 17.89 卤3.25 mm. There was a linear regression and a strong correlation between height and the length of the inferior abdominal aorta of the renal artery in adults of different genders. The regression equation was as follows: the length from the inferior aorta to the bifurcation part of the renal artery in adult males was 0.064 脳 height -1.159 cm. The length of the inferior abdominal aorta to the bifurcation part of the renal artery was 0.084 脳 -3.734 cm in adult female. The regression coefficient was equivalent to the analysis of variance (P < 0.05). The correlation coefficient r was greater than 0.6. There was a linear regression and strong correlation between the height of the same height and the length of the inferior abdominal aorta to the bifurcation part of the bilateral femoral artery. The regression equation was as follows: the length of the bifurcation of the inferior abdominal aorta of the renal artery to the left femoral artery was 0.192 脳 -2.706 cm in adult males. The length of the bifurcation from the inferior aorta of the renal artery to the right femoral artery was 0.200 脳 -3.581 cm in adult males. The length of the bifurcation part of the inferior abdominal aorta to the left femoral artery in adult women was 0.224 脳 -7.414 cm in height. The length of the bifurcation part of the inferior abdominal aorta to the right femoral artery in adult women was 0.314 脳 -20.9 cm, and the regression coefficient was equivalent to T test and ANOVA (P < 0.05). The correlation coefficient r was more than 0.6. There was no linear regression between height and diameter of abdominal aorta (P > 0.05). The anterior and posterior diameter of the inferior abdominal aorta of renal artery was 15.53 卤2.39 mm, the diameter of left and right was 15.03 卤2.09 mm. The diameter of left common iliac artery and right common iliac artery was 8.47 卤1.05mm and 8.59 卤1.17mm. Conclusion accurate understanding of the applied anatomical data of the inferior abdominal aorta of renal artery enriches the physical anatomy data of the abdominal aorta in China. At the same time, it has important guiding significance for emergency endovascular repair and open operation of ruptured abdominal aortic aneurysm.
