【摘要】:第一部分 人工耳蜗植入术相关结构的解剖测量 目的 通过对人工耳蜗植入手术相关的解剖结构进行观察、测量,为人工耳蜗植入手术提供理论参数。 方法 1.1 材料:取经福尔马林浸泡的国人成人头颅湿标本10例20耳,均来自第一军医大学解剖学教研室,其中女性4例8耳,男性6例12耳,均无中耳畸形和疾病(研究中证实)。测量工具:自制仿镫骨测量器,制图用带长针头分规,精确度0.02mm的游标卡尺,耳科莱卡手术显微镜,耳科电钻,常规耳科手术器械。 1.2 方法: 1.2.1 实验步骤:经乳突后鼓室进路,行乳突切除,保留外耳道后壁,暴露砧骨短脚,磨出面神经垂直段(磨去骨管)和鼓索神经,在两者之间切开面隐窝,进入后鼓室,暴露卵圆窗、圆窗龛。在圆窗龛前下方1.422mm处钻孔进入鼓阶。显露水平半规管隆起,磨出蓝线(膜半规管)。在显微镜下观察测量相关解剖标志及数据。然后去除砧骨,在匙突下方1.338mm处钻孔进入耳蜗第二圈,磨出耳蜗底转、第二圈及颈内动脉管,测量耳蜗底转前缘与颈内动脉管后缘之间的距离。 1.2.2 主要观察指标:镫骨头下缘与耳蜗底转切开点之间的距离,镫骨头下缘与圆窗龛之间的距离,圆窗龛与耳蜗底转切开点之间的距离,镫骨头下缘与耳蜗第二转切开点之间的距离,镫骨头下缘与匙突之间的距离,匙突与耳蜗第二转切开点之间的距离,面神经垂直段与圆窗龛的距离,面神经与鼓索神经在圆窗平
[Abstract]:Part I Anatomical measurements of the related structures of cochlear implants Purpose The anatomical structure related to cochlear implant operation was observed and measured to provide theoretical parameters for cochlear implant operation. Method Materials: 10 adult cranial wet specimens, 20 ears from the Department of Anatomy, first military Medical University, were collected from 10 Chinese adults soaked in formalin. Among them, 4 cases (8 ears) were female and 6 cases (12 ears) were males. There were no deformities and diseases of the middle ear (confirmed in the study). Measuring tools: self-made imitating stapes measuring instrument, drawing with long needle gauge, Vernier caliper with an accuracy of 0.02 mm, and otoleca surgical microscope. Ear drill, conventional otologic instruments. 1.2 methods: 1.2.1 Experimental steps: through the posterior tympanic approach, mastoid resection was performed, the posterior wall of the external auditory canal was preserved, the short foot of the incus was exposed, and the vertical segment of the facial nerve (grinding the bone canal) and the chorda tympani nerve were ground out. Cut the recess of the surface between the two, enter the posterior tympanic chamber, expose the oval window, the circle window niche, drill into the drum step at 1.422mm below the circle window niche, and expose the horizontal semicircular canal protuberance. The blue line (membranous semicircular canal) was ground out. The anatomic signs and data were observed under microscope. Then the incus was removed, and the second circle of cochlea was drilled into 1.338 mm below the spassus, and the cochlea was rotated at the bottom of the cochlea. The second circle and the internal carotid artery canal were used to measure the distance between the cochlear bottom and the posterior edge of the internal carotid artery. 1.2.2 main outcome measures: the distance between the inferior edge of the stapes head and the incision point of the cochlear fundus, the distance between the inferior margin of the stapes head and the circle window niche, and the distance between the circle window niche and the cochlear bottom turning incision point. The distance between the inferior edge of the head of the stapes and the second turn of the cochlea, the distance between the inferior edge of the head of the stapes and the spatula, the distance between the spassum and the second turning point of the cochlea, the distance between the vertical segment of the facial nerve and the recess of the round window.
