冻融胚胎移植与胚胎植入相关性的研究

【摘要】:目的:冻融胚胎移植作为辅助生殖技术的有效补充治疗手段,日趋受到关注。冻融胚胎移植周期的子宫内膜准备方案,目前在临床上主要有两种:自然周期和激素替代周期。对于以上两种方案对临床妊娠率的影响国内外学者的观点仍不一致。因此,本研究旨在通过比较两种方案对临床妊娠率的影响,以指导临床选择较好的方案行冻融胚胎移植前子宫内膜准备。因冻融胚胎移植妊娠失败大多发生在胚胎植入阶段,所以子宫内膜对胚胎的接受状态成为目前研究的热点。由于研究内膜容受性的内膜活检和宫腔冲洗技术都有侵入性且对妊娠有消极影响,不宜用于冻融胚胎移植治疗周期。已有许多研究证实细胞因子和生长因子在胚胎植入过程中起重要作用,例如白血病抑制因子(leukemia inhibitory factor,LIF)、表皮生长因子(epidermal growth factor,EGF)、巨噬细胞集落刺激因子(macrophage-colony stimulating factor,M-CSF)等。近来研究表明与胚胎植入相关的细胞因子和生长因子在生殖道分泌物中也可测出,这样采集标本所需技术的侵入性即可减小到最小程度。分泌物中所测得的细胞因子很可能来自子宫内膜,而他们的表达也可能受控制内膜分泌细胞因子的激素所调节。因此,本研究的另一个目的在于通过比较妊娠组与非妊娠组内膜的超声类型与厚度,血清雌二醇(estuarial,E_2)、孕激素(progesterone,P)、雌孕激素比值(estuarial progesterone ratio,E_2/P),血清及宫颈分泌
[Abstract]:Objective: freezing and thawing embryo transfer as an effective supplementary therapy of assisted reproductive technology has attracted more and more attention. At present, there are two main clinical types: natural cycle and hormone replacement cycle. The purpose of this study was to compare the effects of two methods on the clinical pregnancy rate in order to guide the clinical choice of the better protocol for the preparation of endometrium before freeze-thawed embryo transfer. The failure of frozen and thawed embryo transfer pregnancy mostly occurred in the embryo implantation stage. Therefore, the status of endometrial acceptance of embryos has become a hot topic. Because endometrial biopsy and uterine cavity irrigation are both invasive and have a negative impact on pregnancy, It is not suitable for the treatment cycle of freeze-thawed embryo transfer. Many studies have proved that cytokines and growth factors play an important role in embryo implantation. For example, leukemia inhibitory factor, epidermal growth factor, macrophage colony stimulating factor, macrophage colony stimulating factor, and so on. Recent studies have shown that cytokines and growth factors associated with embryo implantation can also be detected in reproductive tract secretions. This minimizes the invasive nature of the techniques needed to collect the samples. The cytokines measured in the secretion are likely to come from the endometrium, and their expression may also be regulated by hormones that control the secretion of cytokines from the endometrium.
