腰椎峡部裂修复重建手术的应用解剖学研究

【摘要】:研究设计: 对成年人L4,5椎骨横突、椎板、峡部及与其前方脊神经前支的关系进行大体解剖学测量,并对L4,5椎骨横突内部结构进行硬组织切片观察。 研究目的: 为腰椎峡部裂的修复重建手术提供解剖学依据,以提高手术的安全性和有效性。 研究背景: 峡部裂的修复重建手术由于能够最大限度保留脊柱的运动功能,更加符合人体的生理状态,故近年来迅速发展。但国内由于缺乏国人的相关应用解剖学数据的指导和支持,临床医师在实施该手术时只能凭借经验进行操作,无法确保操作的精确性和安全性,以及所选用器材尺寸的合理性。 材料与方法: 对31具成年人L4,5椎骨骼标本进行解剖测量,测量指标如下:(1)横突:双侧横突长度,横突起始部宽度,横突内1/4点处宽度,横突中点处宽度,横突起始部厚度,横突内1/4点处厚度,横突中点处厚度。(2)椎板:椎板中部高度,椎板上缘中点处厚度,椎板下缘中点处厚度,椎板中部厚度,椎板与棘突根部交线中点到对侧上关节突起始部前缘的距离,椎板与棘突根部交线中点到对侧横突起始部前缘的距离,椎板与棘突根部交线中点到对侧下关节突起始部前缘的距离,正上方观钉道与矢状面的夹角,正后方观钉道与矢状面的夹角。(3)峡部:峡部中点处的高度,峡部上缘与上关节突交界处的厚度,峡部下缘与下关节突交界处的
[Abstract]:Research and design:. The transverse process, lamina, isthmus and the anterior branch of the anterior spinal nerve were measured and the internal structure of the transverse process was observed by hard tissue section. Objectives of the study:. To provide anatomical basis for the repair and reconstruction of lumbar spondylolysis, in order to improve the safety and effectiveness of the operation. Background:. The repair and reconstruction operation of isthmus bifida has been developed rapidly in recent years because it can preserve the motor function of the spine to the maximum extent and is more in line with the physiological state of the human body. However, there is a lack of guidance and support from the relevant applied anatomical data in China. The clinician can only use experience to perform the operation, which can not ensure the accuracy and safety of the operation, and the rationality of the size of the equipment selected. Materials and methods:. The anatomical measurements were performed on 31 adult L4 / 5 vertebrae specimens. The measurement indexes were as follows: bilateral transverse process length, width of the beginning of transverse process, width of 1 / 4 point of transverse process, width of midpoint of transverse process, thickness of beginning part of transverse process. The height of the middle of the lamina, the thickness of the midpoint of the upper margin of the lamina, the thickness of the middle point of the lower margin of the lamina, the thickness of the middle point of the lamina, the thickness of the middle part of the lamina, the thickness of the middle point of the transverse process, and the thickness of the midpoint of the transverse process. The distance from the midpoint of the intersecting line between the lamina and the spinous process to the front of the origin of the contralateral superior articular process, and the distance between the midpoint of the intersecting line between the lamina and the root of the spinous process and the front edge of the opposite lateral transverse process. The distance from the midpoint of the intersecting line between the lamina and the spinous process to the anterior edge of the starting part of the contralateral inferior articular process, the angle between the upper epigastric snail and the sagittal plane, and the angle between the posterior posterior snail and the sagittal plane. The height of the isthmus at the midpoint of the isthmus. The thickness at the junction of the upper edge of the isthmus with the superior articular process, and the junction of the lower edge of the isthmus with the inferior articular process.
