心肌梗死小型猪围术期模型的构建及内源性血红素氧合酶-1心脏保护作用的实验研究

【摘要】:目的:探讨运用球囊导管封堵中国小型猪冠状动脉来建立心肌梗死模型的方法研究。方法:选用中国小型猪20头,麻醉后经股动脉或颈总动脉置入球囊导管至冠状动脉左前降支(LAD)的第二对角支(D2)远端部位,先行封堵预处理3~4次,然后再封堵LAD的血流120min。观察心电图、心肌酶、心脏二维超声检查及冠状动脉造影。并允许动物被饲养6~8周。结果:存活的18头猪均完成冠状动脉左前降支远端的封堵,心电图显示急性心肌梗死典型图形变化,血浆心肌酶明显升高并呈动态演变;术后1h超声检查出现室间隔前部及前壁心尖局部运动异常,提示成功地建立了急性心肌梗死模型。AMI术后3至5周复查显示左心室前壁、心尖部心室壁和室间隔异常运动;AMI术后存活的17头猪均为陈旧性心肌梗死模型。术后保持封堵的冠状动脉通畅:与开胸法相比更接近人体冠脉溶栓或急诊冠脉血管成形术后心肌梗死的发展状态,具有创伤小、动物生存时间长并易于术后饲养等优点。但有1头猪死于手术后期。另2头猪分别在堵闭60~100min因心室纤颤抢救无效死亡。结论:运用球囊导管封堵冠状动脉LDA可成功建立小型猪心肌梗死模型。 目的:许多需要手术治疗的老年病人都伴随有心肺等重要器官的疾病,使其手术的风险大大增加,不良的心血管事件是围术期发病率和死亡率升高的主要原因。我们利用陈旧性心肌梗死的小型猪,采用开胸手术构建围术期的实验动物模型和观察心脏结构及功能的损害,证明在体及手术结束时实验模型的有效性和稳定性。方
[Abstract]:Objective: to study the method of using balloon catheter to occlude coronary artery of Chinese miniature pig to establish myocardial infarction model. Methods: after anesthesia, 20 Chinese miniature pigs were placed into the distal part of the second diagonal branch of coronary artery via femoral artery or common carotid artery into the second diagonal branch of left anterior descending coronary artery (lad) after anesthesia. The LAD blood flow was then blocked for 120 minutes. Electrocardiogram, myocardial enzyme, two-dimensional echocardiography and coronary angiography were observed. Animals are allowed to be kept for 6 to 8 weeks. Results: the left anterior descending coronary artery was occluded in all of the 18 surviving pigs. The electrocardiogram showed that the typical pattern of acute myocardial infarction was changed, and the plasma myocardial enzyme increased significantly and showed dynamic evolution. At 1 hour after operation, abnormal local movement of anterior ventricular septum and anterior wall was observed, which suggested that acute myocardial infarction model was successfully established. 3 to 5 weeks after AMI, left ventricular anterior wall was demonstrated by reexamination. All the 17 surviving pigs with abnormal ventricular wall and septal movement after AMI were old myocardial infarction models. Compared with open chest method, it is closer to the development of myocardial infarction after coronary thrombolysis or emergency coronary angioplasty, and has the advantages of less trauma, longer survival time and easy to be reared after operation. But one pig died after the operation. The other two pigs died of ventricular fibrillation in occluded 60~100min. Conclusion: the myocardial infarction model of miniature pig can be successfully established by balloon catheter occlusion of coronary artery LDA. Objective: many elderly patients in need of surgical treatment are accompanied by diseases such as heart, lung and other important organs, which greatly increase the risk of surgery. Adverse cardiovascular events are the main reasons for the increase of perioperative morbidity and mortality. In order to prove the validity and stability of the experimental model in vivo and at the end of the operation, we used old myocardial infarction miniature pigs to construct perioperative experimental animal model and observe the damage of cardiac structure and function.
