椎间盘退变动物模型的比较研究

【摘要】: 【研究背景】 椎间盘退变的动物模型是研究椎间盘退变性疾病发病机制的一种重要途径,同时为修复退变椎间盘的治疗研究提供良好的实验载体。许多学者选择不同的动物、通过不同的方法建立了椎间盘退变的动物模型。按退变发生的原因可将其分为两大类,即诱发性和自发性椎间盘退变模型。前者是通过实验的方法诱发椎间盘退变,可按实验方法的不同进一步分为3种:①改变椎间盘生物力学环境诱发退变;②损伤椎间盘自身结构诱发退变;③通过基因敲除技术制作退变模型。其中手术直接损伤型椎间盘退变模型,是目前较为普遍采用的一种建模方法,虽然其已广泛地应用于椎间盘退变的各个研究领域当中,但对不同手术损伤方式所诱导的退变模型进行比较的研究尚少见,对于不同椎间盘退变模型的选择仍有许多不同意见。 【研究目的】 1.建立不同手术损伤方式所诱导的兔椎间盘退变模型,,并比较其退变过程及其特点; 2.为在椎间盘退变性疾病的研究中,更为合理地选择与自然退变过程相似和具有可行性、可重复性的动物模型提供理论依据。 【材料与方法】 本实验选用健康新西兰兔20只,采用随机对照的实验设计方案,使用三种不同的方法(纤维环穿刺、髓核抽吸、终板破坏)建立椎间盘退变的动物模型,分批在术后第2、4、8、16、和32周等时间点处死实验动物并获取椎间盘标本。通过检测髓核组织水和硫酸化糖胺多糖(s-GAG)含量的改变,同时结合影像学检查(CR片测量椎间盘高度,MRI T2WI评估退变程度),对不同方法建立的椎间盘退变动物模型的退变过程及其特点进行观察和比较。所有实验数据应用SPSS 13.0统计学软件,t检验和两两比较的方差分析。 【结果】 根据髓核组织水和硫酸化糖胺多糖(s-GAG)含量的测量结果,同时结合影像学检查(CR片测量椎间盘高度,MRI T2WI评估退变程度)的结果,证实三种手术损伤方式均成功地建立了椎间盘退变模型。同时各项指标均呈现与自然椎间盘退变过程相类似的随时间延长而变化的趋势。但终板破坏和髓核抽吸所诱发的椎间盘退变出现在2周后,而纤维环穿刺所诱发的椎间盘退变则出现在4周左右。 【结论】 终板破坏和髓核抽吸型退变模型所诱发的退变过程出现较早,进展较快且退变程度较重,适合用于对中晚期退变的相关研究。而纤维环穿刺法诱发的退变过程较为缓慢,且退变程度与穿刺损伤程度呈相关性,适合用于对早期退变的相关研究
[Abstract]:[research background] The animal model of disc degeneration is an important way to study the pathogenesis of degenerative disc disease and provides a good experimental carrier for the treatment of degenerative disc repair. Many scholars choose different animals and establish animal models of disc degeneration by different methods. According to the causes of degeneration, it can be divided into two categories: induced and spontaneous disc degeneration models. The former can be further divided into 3 kinds according to the different experimental methods, which are induced by experimental method, and can be further divided into 3 kinds: 1. Change the biomechanical environment of intervertebral disc to induce degeneration. 3 the degenerative model was made by gene knockout technique. Surgical direct injury model of disc degeneration is a widely used modeling method, although it has been widely used in various research fields of disc degeneration. However, it is rare to compare the models of degeneration induced by different surgical injury methods, and there are still many different opinions on the choice of different models of intervertebral disc degeneration. [purpose of research] 1. The degenerative model of rabbit intervertebral disc induced by different surgical injury methods was established, and the process and characteristics of degeneration were compared. 2. It provides a theoretical basis for selecting more reasonable animal models which are similar to natural degeneration in the study of degenerative intervertebral disc disease and are feasible and reproducible. [materials and methods] In this experiment, 20 healthy New Zealand rabbits were used to establish the animal model of intervertebral disc degeneration using three different methods (annular puncture, aspiration of nucleus pulposus, destruction of endplate). The experimental animals were killed and the intervertebral disc specimens were collected at the 2nd week and the 32nd week after operation. The changes of water and sulfated glycosaminoglycan (S-GAG) contents in nucleus pulposus were measured. At the same time, the degenerative degree of intervertebral disc was evaluated by MRI T2WI combined with CR film. The degenerative process and characteristics of the animal model of degenerative intervertebral disc were observed and compared. All experimental data were tested by SPSS 13.0 statistical software / t test and anatomic analysis of variance (ANOVA) of pairwise comparison. [results] According to the results of measurement of water and sulfated glycosaminoglycans in nucleus pulposus tissue, combined with imaging examination, the degree of degeneration was evaluated by MRI T2WI of intervertebral disc height measured by CR film. It was proved that the models of intervertebral disc degeneration were established successfully in all three kinds of surgical injury modes. At the same time, all indexes showed a trend of change with time similar to the process of degeneration of natural intervertebral disc. However, disc degeneration induced by endplate destruction and aspiration of nucleus pulposus occurred 2 weeks later, while disc degeneration induced by fiber ring puncture appeared at about 4 weeks. [conclusion] The degenerative process induced by end-plate destruction and aspiration degeneration model of nucleus pulposus appeared earlier, advanced quickly and had a severe degree, so it was suitable for the study of degeneration in the middle and late stages. However, the process of degeneration induced by fiber ring puncture is slow, and the degree of degeneration is correlated with the degree of puncture injury, which is suitable for the study of early degeneration.
