糖皮质激素和生长激素对内毒素损伤中性粒细胞凋亡的影响

【摘要】:[目的] 目前脓毒症和脓毒症性休克仍然是危重病人的主要死亡原因之一,特别是在婴幼儿由于其自身发育特点,脓毒症和脓毒症性休克的死亡率更是居高不下。传统的治疗方法并不能明显改善其预后,降低死亡率。进一步探讨其发病机理,寻求有效的治疗方法是十分必要的。大量研究认为中性粒细胞在脓毒症时发生凋亡延迟,导致炎症反应过度,造成组织损伤是脓毒症发展恶化的原因之一。以往研究认为糖皮质激素具有拮抗内毒素的作用,但最近研究发现体外条件下糖皮质激素对中性粒细胞的凋亡有延迟作用。那么,究竟糖皮质激素在脓毒症时对内毒素损伤后的中性粒细胞有何作用,是保护还是损伤?这是本实验的目的之一。生长激素是一种蛋白质激素,以往一直被用来治疗垂体性侏儒,但最近许多文献报道生长激素在危重症中有积极的作用,能提高脓毒症动物的生存率。那么生长激素对内毒素损伤后中性粒细胞的凋亡是否也存在影响,这一影响在脓毒症中的意义如何呢?这是本实验的另一个目的。我们希望通过本实验为临床进一步探讨脓毒症和脓毒症性休克的治疗提供参考。 [方法] 采用不连续密度梯度离心法分离人中性粒细胞于CO_2培养箱(37℃,5%CO_2)中培养过夜。实验共分8组(n=3),分别为正常对照组(N),内毒素组(LPS),大剂量地塞米松组(DEX1),大剂量地塞米松治疗组(LPS+DEX1),小剂量地塞米松组(DEX2),小剂量地塞米松治疗组(LPS+DEX2),生长激素组(GH),生长激素治疗组(LPS+GH),其中LPS的终浓度为10ug/ml,DEX1的终浓度为10~(-6)M,DEN2的终浓度为10~(-8)M,GH的终浓度为100ug/ml。每组均以3小时,6小时,12小时,24小时作为检测时点。根据实验设计于96
