胆囊三角及胆囊动脉应用解剖学研究

【摘要】： 胆囊三角(Calot三角)是指由胆囊管、肝总管和肝的脏面围成的三角形区域。近年来腹腔镜胆囊切除术(LC laparoscope cholecystectomy)广泛开展,传统开腹胆囊切除手术(OC)已被腹腔镜胆囊切除手术所替代。在腹腔镜下行胆囊切除时,胆囊三角的分离是最关键的。胆囊三角内有胆囊动脉通过,胆囊动脉的起始、分支、走行,存在个体性差异,并且该处空间狭小,与周围组织结构紧密,因此胆囊动脉、胆管的损伤大部分发生在这里。为了最大限度地避免不必要的并发症发生,就需要熟悉胆囊三角的应用解剖及胆囊动脉的变异。本文利用26具防腐成人尸体标本,解剖观察胆囊三角的形态,对重要的结构进行量化分析,并对胆囊动脉的起始、分支、走行及胆囊管、肝总管的汇合方式及位置分型的临床意义等问题做了进一步阐述,以期为手术中防止胆囊动脉、胆囊管的损伤提供解剖学依据,应用于临床实践。
[Abstract]:The Calot Triangle of the gallbladder is a triangular region surrounded by the duct of the gallbladder, the common hepatic duct, and the visceral surface of the liver. In recent years, laparoscopic cholecystectomy (LC laparoscope cholecystectomy) has been widely carried out, and traditional open cholecystectomy has been replaced by laparoscopic cholecystectomy. In laparoscopic cholecystectomy, the separation of the gallbladder triangle is the most critical. In the triangle of gallbladder, there are gallbladder arteries passing through, the beginning, branches, and running of the gallbladder arteries, there are individual differences, and the space there is narrow and close to the surrounding tissue structure, so most of the injuries of the cholecystic artery and bile duct occur here. In order to avoid unnecessary complications, it is necessary to know the applied anatomy of gallbladder triangle and the variation of gallbladder artery. In this paper, 26 antiseptic adult cadavers were dissected to observe the shape of the gallbladder triangle, the important structures were quantitatively analyzed, and the origin, branch, running and gallbladder duct of the gallbladder artery were analyzed. The confluence of the common hepatic duct and the clinical significance of the location classification were further expounded in order to provide anatomic basis for preventing the injury of the gallbladder artery and the gallbladder duct during the operation and to apply it to clinical practice.
