护理本科实习生心理健康状况及相关因素的纵向研究

【摘要】: 目的:通过不同时间点对本、专科护生应激性事件及心理健康状况进行跟踪测评,探讨护理本科实习生心理状况变化的特点及其客观影响因素,为及时发现护生的心理问题、有的放矢地对其进行心理健康教育和心理问题的干预提供科学依据。 方法:①采用随机抽样的方法,抽取湖南、山东两地两所医学院校护理专业2007届毕业生共295人,本科组165人和专科组130人。采用大学毕业生应激性事件量表(CSSS)和症状自评量表(SCL-90)进行5次历时近8个月的跟踪调查。所得数据采用SPSS13.0软件包,应用两样本t检验分析护生心理健康状况与全国青年常模的差异,应用重复测量的方差分析、SNK-q检验等方法,分析本、专科人群心理健康状况的变化特点;应用x~2检验分析本专科护生不同时点心理问题发生率的差异;②采用Pearson相关分析及逐步回归,分析实习不同阶段应激性事件频率变化与心理健康状况变化的关系。 结果:①两样本t检验结果显示,本科护生在实习期间不同测试时点,,SCL-90各因子分低于全国18~29岁青年常模(p<0.05或0.01)或与全国青年常模比较差异无统计学意义(p>0.05);②x~2检验结果显示,在T5、T7时点专科生的心理问题发生率高于本科生(p<0.05);③重复测量方差分析结果显示,不同测试时点护生的心理健康状况不同(p<0.05),本科组与专科组之间存在组间差异,并且分组与时间之间存在交互作用;④两样本t检验结果显示,本科护生与专科生在T0时点与T1时点SCL-90总分差异无统计学意义(p>0.05),而在T3、T5、T7时点,SCL-90总分本科组低于专科组(p<0.05);⑤q检验结果表明,本科护生在T3时点SCL-90总分最低,T3时点与T0、T1、T5、T7之间差异均有统计学意义(p<0.05),而T0、T1、T5、T7两两之间差异无统计学意义(p>0.05);专科护生T5、T7时点SCL-90总分与T0、T1、T3之间两两比较,差异有统计学意义(p<0.05),T5与T7之间,以及T0、T1、T3两两之间SCL-90总分之间的差异无统计学意义(p>0.05);⑥Pearson相关分析结果显示,在实习的不同阶段,大多数应激性事件频率的变化与SCL-90总分变化之间存在显著正相关(p<0.05);⑦逐步回归结果显示,第一阶段,人际关系的变化进入了回归方程(p<0.01);第二阶段,人际关系压力及日常生活困扰的变化依次进入了回归方程(p<0.01);第三阶段,人际关系压力及学习压力依次进入了回归方程(p<0.01或0.05);第四阶段,求职压力与日常生活困扰依次进入了回归方程(p<0.01或0.05)。 结论:①与全国青年常模相比,本科护生在不同测试时点心理健康状况较好;②在不同的测试时点,本专科护生心理健康状况不同,本科护生好于专科生;③本科实习护生从实习前两周到实习第七个月末,心理健康水平逐渐上升又逐渐下降至与实习前水平持平,而专科护生的心理健康水平先保持平稳又逐渐下降且低于实习前水平;④不同实习阶段影响护生心理健康状况变化的因素不同;⑤重视护生实习前与择业期的心理健康教育,帮助护生正确处理人际关系,使其确立正确的职业生涯规划。
[Abstract]:Objective: To evaluate the stressful events and mental health status of the nursing students at different time points, to explore the characteristics and objective factors of the changes in the psychological status of the nursing undergraduates, and to find out the psychological problems of the nursing students in time and to carry out the psychological health education and psychological problems in a definite direction. Learn the basis.
Methods: (1) a total of 295 nursing students from two medical colleges and universities in Hunan and Shandong were selected by random sampling, and 165 students and 130 college students in the undergraduate group were enrolled in this study. The students' stress event scale (CSSS) and the symptom checklist (SCL-90) were used to carry out a follow-up survey of 5 times for nearly 8 months. The data were used in SPSS13.. 0 software package, using two samples t test to analyze the difference between the mental health status of the nursing students and the National Youth norm, the analysis of variance and SNK-q test of repeated measurement, the characteristics of the psychological health status of this special group, and the difference of the incidence of psychological problems at different points of the nursing students by x~2 test; and the use of Pe Arson correlation analysis and stepwise regression analysis were conducted to analyze the relationship between frequency of stressful events and mental health in different stages of internship.
Results: (1) the results of two samples t test showed that there was no significant difference between the SCL-90 factor scores of the national 18~29 year old youth norm (P < 0.05 or 0.01) or the National Youth norm (P > 0.05) during the different test time points during the practice period of undergraduate nursing students (P > 0.05). (2) the result of x~2 test showed that the psychological problems of junior college students were higher in T5 and T7. The results of the repeated measurement of variance analysis showed that the psychological health of the nursing students at different time points was different (P < 0.05), and there was a difference between the group and the specialist group, and there was a interaction between the group and the time. (4) the t test of two samples showed that the undergraduate nursing students and the junior college students were at the time point of T0 and the time point of T1. The total score of SCL-90 was not statistically significant (P > 0.05), while in T3, T5 and T7, the total score of SCL-90 in the undergraduate group was lower than that of the specialist group (P < 0.05). 5. The results of Q test showed that the total score of SCL-90 in the undergraduate nursing students was the lowest at T3 time point, and there were statistical significance between T3 time points and T0, wasting, and 22. P > 0.05); the difference was statistically significant (P < 0.05), between T5 and T7, and between T7, and T0, T1, and 22 (0.05). (0.05) the difference between SCL-90 and T0, T1 and T3 was statistically significant (0.05). There was a significant positive correlation between the change of rate and the change of SCL-90 total score (P < 0.05); and stepwise regression results showed that the first stage, the change of interpersonal relationship entered the regression equation (P < 0.01); the second stage, the changes of interpersonal pressure and daily life distress entered the regression equation (P < 0.01) in turn, and the third stage, the interpersonal relationship pressure. The stress and learning pressure entered the regression equation in turn (P < 0.01 or 0.05). In the fourth stage, the pressure of job hunting and daily life plagued the regression equation (P < 0.01 or 0.05) in turn.
Conclusion: compared with the National Youth norm, the mental health status of the undergraduate nursing students at different time points is better. 2. At different time points, the mental health status of the nursing students is different, the undergraduate nursing students are better than the junior college students, and the mental health level of the undergraduate nursing students from two weeks to the end of the seventh month of the internship, the level of mental health is gradually rising and running. Gradually decreased to the level before the practice, and the mental health level of the specialized nursing students remained stable and decreased gradually and lower than the pre practice level; (4) the factors affecting the changes of mental health status of the nursing students were different. To establish a correct career plan.
