胸廓内动脉的超声应用解剖学研究

【摘要】：目的通过对胸廓内动脉的解剖学观测及与彩色多普勒超声的对照研究,旨在为心冠状动脉桥接术和影像解剖学提供依据和解剖学资料。方法选用成人尸体标本30例,其中男性20例,女性10例,观测其ITA和冠状动脉;选取131例正常成人其中男性78例,女性53例,平均年龄50.4±6.7岁;采用实验仪器为GE彩色超声仪(美国GE公司制造),设置仪器于血管条件,探头频率为10～12MHz,彩色多普勒频率2～4MHz;检查方法:于胸骨旁和锁骨上窝测量ITA的内径和血流参数,包括Vmax,Vmin,Vd,TAMAX,RI,PI,统计分析:采用SPSS11.5版统计软件,各测量数据采用均数±标准差表示。各组之间的比较用t检验。结果解剖学研究表明:胸廓内动脉自起点至终端长度均值为16.04cm;左侧胸廓内动脉外径在起始处、第二段、第三段和终端处均值分别为2.8mm、2.4mm、2.2mm和2.0mm,右侧胸廓内动脉外径在起始处、第二段、第三段和终端处均值分别为2.7mm、2.3mm、2.1mm和1.9mm;冠状动脉前室间支、旋支、左缘支、对角支、右冠状动脉、后室间支外径均值分别为3.5mm、3.0mm、1.9mm、2.0mm、3.5mm和2.1mm。彩色多普勒超声显示:左右侧ITA各段内径、Vmax、Vmin、Vd、TAMAX、
[Abstract]:Objective to provide basis and anatomical data for coronary artery bridging and image anatomy by observing the anatomy of internal thoracic artery and comparing it with color Doppler ultrasound. Methods the ITA and coronary arteries were observed in 30 adult cadavers (20 males and 10 females) and 131 normal adults (78 males and 53 females, mean age: 50.4 卤6.7 years). GE color ultrasound instrument (made by GE Company of USA) was used to set up the instrument in vascular condition, the probe frequency was 10 ~ 12 MHz, and the color Doppler frequency was 2 ~ (4) MHz. Methods: the diameter and blood flow parameters of ITA were measured near sternum and supraclavicular fossa, and the blood flow parameters of ITA were measured in the sternum and supraclavicular fossa. The statistical analysis: SPSS 11.5 software was used, and the measured data were expressed as mean 卤standard deviation. T test was used to compare each group. Results Anatomical studies showed that the mean length of the internal thoracic artery was 16.04 cm, the external diameter of the left internal thoracic artery was 2.8mm, 2.4mm 2.2mm and 2.0mm respectively at the beginning, and the external diameter of the right internal thoracic artery was at the beginning. The mean values of the second, third and terminal segments were 2.7mm / 2.3mm / 2.1mm and 1.9mm, respectively, and the external diameters of anterior interventricular branch, circumflex branch, left marginal branch, diagonal branch, right coronary artery and posterior interventricular branch were 3.5mm ~ 3.0mm ~ 1.9mm ~ 2.0mm ~ 3.5mm and 2.1 mm respectively.
