周围神经功能储备的实验研究

【摘要】: 周围神经作为一种组织器官,与其他组织器官一样具有一定的功能储备,这种功能储备为神经损伤后的功能恢复提供了代偿能力,从而为临床应用部分神经束支移位修复周围神经损伤的可行性提供了生理解剖学依据。本项目利用神经显微解剖、DiI神经束路示踪及神经电生理学等检测技术研究大鼠主要神经分支纤维在周围神经干内不同平面(纵向)、不同部位(横向)的功能定位情况、分布规律;探讨周围神经功能储备量及损伤后的代偿能力;确定供体部分神经束移位时最佳切取平面、切取部位和最大切取比例的量化指标,用于解决臂丛神经根性撕脱伤及在神经缺损行神经移位时动力神经源不足的问题。为临床更加有效、合理地应用部分神经束支移位术治疗周围神经损伤提供可靠的理论和实验依据,为扩大神经移位术临床应用范围设计出科学可行的标准化手术方法。
[Abstract]:As a kind of tissue and organ, peripheral nerve has certain function reserve, which provides compensatory ability for functional recovery after nerve injury. It provides the physiological and anatomical basis for clinical application of partial nerve bundle branch transposition to repair peripheral nerve injury. The purpose of this project was to study the functional localization of the main nerve branching fibers in different planes (longitudinally) and at different positions (transverse) in the peripheral nerve trunk by means of nerve microanatomy and nerve tracer and neuroelectrophysiology. The distribution law, the quantity of peripheral nerve function reserve and the compensatory ability after injury, and the quantification index of the optimal cutting plane, the cutting position and the maximum cutting proportion of donor nerve bundle were determined. It is used to solve the problem of brachial plexus root avulsion injury and lack of dynamic nerve source during nerve transfer of nerve defect. To provide a reliable theoretical and experimental basis for the treatment of peripheral nerve injury with partial nerve bundle branch transposition, and to design a scientific and feasible standardized surgical method for expanding the scope of clinical application of nerve transposition.
