鼠声带损伤动物模型的建立及修复早期声带Ⅲ型胶原和纤维粘连蛋白的表达

【摘要】: 背景和目的:继发于感染、损伤和外科手术的声带瘢痕(vocal fold scar)是声带受损后引起音质改变的主要原因。研究提示:声带瘢痕一般发生在声带的固有层(lamina propria,LP)。声带固有层主要由纤维蛋白、糖蛋白和蛋白多糖等多种细胞外基质(extracellular matrix,ECM)组成,细胞外基质组成物的含量、性质及各组成物之间的相互作用对正常声带生物机械特性的维持具有重要意义。本实验使用大鼠构建声带机械损伤动物模型,运用免疫组化染色,观察修复早期不同时间声带固有层中Ⅲ型胶原(collagenⅢ)和纤维粘连蛋白(fibronectin,FN)的表达及分布变化规律。 材料和方法:4—6个月的SD大鼠45只,雌雄不限,平均体重约420g。实验组40只在直径1.9mm鼻内窥镜的辅助下,行双侧声带表面被覆层剥脱术,术后死亡4只,将存活的36只随机分为A、B、C、D4组,平均每组9只,于术后不同时间(3天、5天、7天、14天)获取全喉标本。5只为正常对照E组,在实验之初即处死获取全喉标本。所有标本经10%中性甲醛固定,石蜡包埋,4μm连续切片,用免疫组化染色检测Ⅲ型胶原和纤维粘连蛋白的表达,采用HPIAS-100高清晰度彩色病理图文分析系统将染色不同深浅度转化为灰度数值,作定量检测。将上述四个实验组所得灰度数值及对照组进行统计学分析,,统计学采用单因素方差分析中的Dunnett-t,SNK-q检验,所有数据输入SPSS13.0统计学软件进行处理。 结果:1.HE染色:术后3天的标本HE染色显示正常声带粘膜上皮及固有层缺失,新鲜肉芽组织形成并有少量胶原纤维表达;5天受损区域上皮爬行并增生形成复层结构,新鲜肉芽组织、胶原大量形成;7天上皮爬行基本完成,LP内再生胶原组织形成粗大束状结构;14天损伤区上皮变薄呈单层上皮结构,LP内胶原组织重排。 2.免疫组化染色:对照组中Ⅲ型胶原主要分布于固有层浅层和深层。纤维粘连蛋白主要分布于基底膜区域,少量表达于固有层浅层。实验组Ⅲ型胶原和纤维粘连蛋白14天内各时间段两者表达都与对照组有显著性差异(P<0.05),但各实验组之间表达无差异(P>0.05)。表达范围扩大至整个固有层,表达不均匀,呈斑片状,固有层内细胞增多。且Ⅲ型胶原和纤维粘连蛋白的表达呈正相关(γ=0.667,P=0.001)。 结论:1.尽管大鼠声带较小,但在内窥镜辅助下能够准确损伤大鼠声带被覆层,构建大鼠声带损伤动物模型,观察大鼠声带瘢痕形成过程,为声带损伤及声带瘢痕的形成过程的深入研究提供有利条件。 2.急性声带损伤早期Ⅲ型胶原和纤维粘连蛋白表达均增高,分布区域至固有层全层;且二者在固有层中的表达有一定相关性,说明纤维粘连蛋白在声带固有层中导向胶原沉积的模板作用。Ⅲ型胶原14天内维持高水平恒定表达。
