胆碱能系统参与SCS镇痛作用机制的实验研究

【摘要】:本研究通过建立与人类神经源性疼痛高度相近的坐骨神经损伤致疼痛模型,根据坐骨神经损伤后大鼠疼痛行为学的变化以及对脊髓刺激(SCS)反应的有效性或无效性进行分组,应用微量分析方法、高压液相色谱分析法和拮抗剂阻断实验观察SCS过程中脊髓后角内乙酰胆碱浓度的变化以及SCS激活的乙酰胆碱受体亚型情况,以初步判定脊髓胆碱能神经系统在SCS中的作用;同时利用免疫组织化学方法检测不同组实验动物脊髓内胆碱能受体亚型及相关递质5-HT表达的变化,进一步探讨其在SCS中的作用。结果表明,脊髓内胆碱能神经系统与SCS镇痛机制密切相关,尤其是M4型毒蕈碱受体;脊髓后角内Ach的水平与脊髓刺激有效性成正相关。本研究为临床应用SCS治疗神经源性疼痛提供了更深度的理论依据和治疗靶点。
[Abstract]:In this study, the pain model of sciatic nerve injury was established, which was similar to that of human neuropathic pain, and was divided into groups according to the changes of pain behavior and the response of (SCS) to spinal cord stimulation in rats after sciatic nerve injury. The changes of acetylcholine concentration in the posterior horn of spinal cord and the acetylcholine receptor subtypes activated by SCS during SCS were observed by microanalysis, high pressure liquid chromatography and antagonist blocking experiments. The role of spinal cholinergic nervous system in SCS was preliminarily determined. The expression of cholinergic receptor subtypes and related transmitters 5-HT in spinal cord of different groups of experimental animals were detected by immunohistochemical method, and the role of cholinergic receptor subtypes in SCS was further discussed. The results showed that the cholinergic nervous system in the spinal cord was closely related to the analgesic mechanism of SCS, especially the type M4 muscarinic receptor, and the level of Ach in the posterior horn of the spinal cord was positively correlated with the effectiveness of spinal cord stimulation. This study provides a deeper theoretical basis and therapeutic target for the clinical application of SCS in the treatment of neuropathic pain.
