PAF-AH与过敏性紫癜肾脏损害的关联性研究

【摘要】:目的通过检测血小板活化因子乙酰水解酶(PAF-AH)在过敏性紫癜(HSP)及紫癜性肾炎(HSPN)中的表达,探讨PAF-AH与HSP肾脏损害的关联性,了解HSP患儿急性期血小板活化、血管内皮损伤及凝血状态的相关性,及其与肾脏损害的关系,,为研究HSP的发病机制和临床治疗提供一定的理论依据。 方法收集2010年10月-2011年4月在宁夏医科大学总医院儿科住院的初发过敏性紫癜患儿55例,并按有无肾脏损害将其分为非紫癜性肾炎(非HSPN)组30例和紫癜性肾炎(HSPN)组25例。另选体检健康的40例儿童作为健康对照组。采用实时荧光定量PCR技术检测PAF-AH的mRNA表达,同时采用双抗体夹心酶联免疫吸附法(ELISA)检测血清PAF-AH、血管内皮生长因子(VEGF)、热休克蛋白70(HSP70)蛋白水平变化。将PAF-AH与VEGF、HSP70水平进行相关性分析。同时检测血常规及凝血全套。检测数据采用SPSS13.0软件进行处理。 结果1.紫癜性肾炎组PAF-AH mRNA及蛋白表达较非紫癜性肾炎及健康对照组患儿增高,差异有统计学意义(P0.01);非紫癜性肾炎组PAF-AH蛋白表达较健康对照组患儿增高,差异有统计学意义(P0.01);2.紫癜性肾炎组及非紫癜性肾炎组患儿血清VEGF、HSP70表达均较健康对照组增高,差异有统计学意义(P0.01);紫癜性肾炎组VEGF、HSP70表达又高于非紫癜性肾炎组,差异有统计学意义(P0.01);3.紫癜性肾炎组患儿PAF-AH与VEGF及PAF-AH与HSP70表达均呈正相关(P0.01)。4.紫癜性肾炎组及非紫癜性肾炎组PLT、MPV、PDW、PTA、PTR、INR、APTT-R差异有统计学意义,其中除MPV、PDW和PTA在紫癜性肾炎组增高外,其他指标紫癜性肾炎组较非紫癜性肾炎组均下降。 结论1.PAF-AH参与了HSP的免疫发病机制,并与肾脏损害呈正相关。2.血小板活化、血管内皮损伤及凝血状态与急性期HSP肾脏损害的发生发展密切相关
[Abstract]:Objective to investigate the relationship between PAF-AH and renal damage of HSP by detecting the expression of PAF-AH in Henoch-Schonlein purpura (HSP) and Henoch-Schonlein purpura nephritis (Henoch-Schonlein purpura nephritis). The relationship between vascular endothelial injury and coagulation status, and the relationship between vascular endothelial injury and renal damage, provides a theoretical basis for the study of the pathogenesis and clinical treatment of HSP. Methods 55 cases of primary Henoch-Schonlein purpura were collected from October 2010 to April 2011 in the pediatrics department of Ningxia Medical University General Hospital. The patients were divided into two groups according to whether they had renal damage or not: 30 cases of non-Henoch-Schonlein purpura nephritis group and 25 cases of Henoch-Schonlein purpura nephritis group. 40 healthy children were selected as healthy control group. The mRNA expression of PAF-AH was detected by real-time fluorescence quantitative PCR. The levels of serum PAF-AH, vascular endothelial growth factor (VEGF) and heat shock protein 70 (HSP70) were detected by double antibody sandwich enzyme linked immunosorbent assay (Elisa). The correlation between PAF-AH and VEGF HSP70 was analyzed. The data are processed by SPSS13.0 software. Results 1. The expression of PAF-AH mRNA and protein in Henoch-Schonlein purpura nephritis group was significantly higher than that in non-purpura nephritis group and healthy control group (P 0.01), and the expression of PAF-AH protein in non-purpura nephritis group was higher than that in healthy control group. The expression of VEGF HSP70 in children with Henoch-Schonlein purpura nephritis and non-Henoch-Schonlein purpura nephritis was significantly higher than that in healthy controls (P 0.01), and the expression of VEGF HSP70 in Henoch-Schonlein purpura nephritis was higher than that in non-Henoch-Schonlein purpura nephritis. The expression of PAF-AH and VEGF, PAF-AH and HSP70 in Henoch-Schonlein purpura nephritis (HSPN) group were all positively correlated with that in Henoch-Schonlein purpura nephritis group and non-purpura nephritis group. There was a significant difference in the expression of PAF-AH and VEGF and PAF-AH and HSP70 in patients with Henoch-Schonlein purpura nephritis and non-purpura glomerulonephritis, and there was a significant difference in the expression of PAF-AH and PTA in Henoch-Schonlein purpura glomerulonephritis. Other indexes of Henoch-Schonlein purpura nephritis group were lower than that of non-purpura nephritis group. PAF-AH is involved in the immune pathogenesis of HSP, and is positively correlated with renal damage. 2.Platelet activation, vascular endothelial injury and coagulation are closely related to the development of renal damage in acute HSP.
