中国青少年体重有关生命质量量表条目发展的定性研究

【摘要】：目的我国肥胖及超重青少年的生命质量研究开展较晚,目前还缺乏相应的工具。本研究的目的在于对原有体重有关生命质量量表进行汉化,采用定性研究半结构访谈法了解我国青少年独特的与体重有关的生活感受,发展新条目,研制YQOL-W量表的中文版初稿,为量表内容效度提供重要的证据。方法根据华盛顿大学卫生服务系关于量表翻译过程的指南,通过前译、后译得到量表初始版本,并在目标人群中进行两轮认知访谈,对条目语言及用词进行修改使之适用于青少年,对量表进行文化调适。运用定性研究经典理论“扎根理论”,采用半结构访谈法,对杭州市不同地区、不同BMI指数的11-18岁青少年进行访谈。访谈进行现场录音,并逐字转录。应用Atlas ti5.7.1软件对数据进行编码分析。发现新概念,设计新条目,对新条目进行认知访谈。将修改后的新条目与已有量表相结合,组成青少年体重有关生命质量量表的测试版。结果量表大部分条目在翻译的时候并无困难,可以用中文精确表达,根据两轮认知访谈的结果,对部分条目进行了修正。访谈杭州市22名来自城市、农村和移民群体的肥胖、超重、正常的青少年,从22个转录稿中总共抽取出129个与体重有关的片段、句子、段落,其中19条分类为“自我”领域(14.7%)；54条分类为“社会”领域(41.9%)；56条分类为“环境”领域(43.4%)。其中14条文本与源量表概念不匹配,根据内容独特性以及代表性等标准撰写了5个新条目。在目标人群中对新条目进行三轮认知访谈,反复修改,使条目内容反映概念,文字表达符合青少年的特点。结论本研究运用扎根理论,发展适用于中国青少年、具有文化敏感性的体重特异性生命质量评估工具,提供了量表内容效度的证据。进一步的研究将评价量表的测试性能。量表的研制有助于评估我国青少年与体重有关的生活体验,以及作为体重管理和干预的结局指标。
[Abstract]:Objective to study the quality of life of obese and overweight adolescents in China. A qualitative semi-structured interview method was used to understand the unique life experience of Chinese adolescents related to weight, to develop new items, and to develop the first draft of the Chinese version of the YQOL-W scale, which provided important evidence for the validity of the scale content. Methods according to the guidelines of the Department of Health Services of the University of Washington on the translation process of the scale, the initial version of the scale was obtained through pre-translation and post-translation, and two rounds of cognitive interviews were conducted among the target population. To modify the language and words of items to apply to teenagers, to adapt the scale to culture, to apply qualitative research to the "rooted theory" of classical theory, and to use semi-structured interview method to study different areas of Hangzhou. Interviews were conducted among 11-18 year olds with different BMI indices. Interviews were recorded live and verbatim transcribed. Data were encoded and analyzed using Atlas ti5.7.1 software. New concepts were discovered and new entries were designed. A cognitive interview was conducted on the new items. The revised new items were combined with the existing scales to form a test version of the adolescent weight-related quality of life (QOL) scale. Results most of the items in the scale were not difficult to translate and could be accurately expressed in Chinese. According to the results of two rounds of cognitive interviews, some items were revised. 22 people from urban, rural and immigrant groups were interviewed in Hangzhou. Overweight, normal teenagers, out of a total of 129 body weight related fragments, sentences, paragraphs from 22 transcripts, Of these, 19 were classified as "self" and 54 were classified as "social", and 56 were classified as "environment", and 14 of them did not match the concept of source scale. According to the criteria of uniqueness and representativeness of content, five new items were written. Three rounds of cognitive interviews were carried out among the target population to make the contents reflect the concept and the text expression accorded with the characteristics of teenagers. Conclusion using the rooted theory, this study developed a culturally sensitive weight specific quality of life assessment tool for Chinese adolescents. Further research will evaluate the test performance of the scale. The development of the scale will help to evaluate the life experience of Chinese adolescents related to weight, as well as the outcome of weight management and intervention.
