围产儿先天性心脏病影响因素的流行病学研究

【摘要】:目的:探讨先天性心脏病发病的影响因素,为预防先心病的发生提供科学依据。 方法:采用病例对照研究方法。选择湖南省十四个市州45所国家定点三网监测医院的2007年7月1日--2008年3月31日期间经临床与彩超确诊的253名先心病为病例,按照配对条件(围产儿性别相同,月龄相差2周内),选取同期非先心病的正常围产儿作为对照。病例与对照的父母均采用统一的调查表,由医院妇产科医生当面询问填写,内容包括:产妇基本情况、新生儿基本情况、孕妇怀孕前或孕期情况、家族史、丈夫情况、孕妇症状自评量表。数据采用Epidata3.2进行录入。资料采用统计描述等方法分析母亲和围产儿的一般情况,用SPSS16.0统计软件包进行单因素分析和多因素Logistic回归分析。 结果:本研究单因素分析结果显示孕产妇年龄、孕次、文化程度、职业、孕产妇职业接触危险因素、患慢性病、孕期感冒、早孕反应、孕前或孕期生殖系统感染、居住地有环境污染源、孕期吃泡菜腌熏类食品、吃鱼虾肉蛋类食品、蔬菜水果类、牛奶豆类、孕期工作、孕产妇异常生育史、抑郁、焦虑及丈夫年龄、患慢性病等与先心病的发生有关联;多因素条件Logistic回归分析结果显示:母亲年龄,文化程度,生殖感染,孕期食肉类、鱼、虾类和蛋类,食蔬菜和水果,抑郁,焦虑与先心病的发生有关联。 结论:本研究显示母亲年龄超过35岁,有生殖道感染、情绪抑郁、焦虑是围产儿先心病的危险因素。母亲文化程度高、孕期食肉类、鱼、虾类和蛋类,食蔬菜和水果有利于降低先心病的患病风险。加强孕妇的健康宣教,增强孕妇的营养补充,保持孕期的心情舒畅,避免感染,有利于降低围产儿先心病发生的危险性。
[Abstract]:Objective: to explore the influencing factors of congenital heart disease and to provide scientific basis for prevention of congenital heart disease. Methods: a case-control study was conducted to select 253 patients with congenital heart disease (CHD) diagnosed by clinical and color Doppler ultrasonography from July 1st 2007 to March 31st 2008 in 45 state designated three nets surveillance hospitals in 14 cities and prefectures of Hunan Province. According to the matching conditions (the perinatal sex was the same and the age difference was within 2 weeks), the normal perinatal infants with non-congenital heart disease in the same period were selected as controls. The parents of both cases and controls used a unified questionnaire, which was filled out by the hospital gynaecologist in person. The contents include: the basic situation of the parturient, the basic condition of the newborn, the condition of the pregnant woman before or during pregnancy, the family history, the condition of the husband, The data were inputted by Epidata3.2. The general situation of mother and perinatal was analyzed by statistical description. Univariate analysis and multivariate Logistic regression analysis were carried out with SPSS16.0 software package. Results: univariate analysis showed that maternal age, pregnancy, education, occupation, occupational exposure risk factors, chronic diseases, cold during pregnancy, early pregnancy reaction, infection of reproductive system before or during pregnancy, Environmental pollution sources include eating pickled vegetables and smoked foods during pregnancy, eating fish and shrimp meat and eggs, vegetables and fruits, milk and beans, working during pregnancy, abnormal birth history of pregnant women, depression, anxiety and husband's age. Multivariate conditional Logistic regression analysis showed that the age of mother, education level, reproductive infection, carnivore during pregnancy, fish, shrimp and eggs, vegetables and fruits, depression, etc. Anxiety is associated with congenital heart disease. Conclusion: this study showed that mothers over 35 years old, with reproductive tract infection, depression and anxiety were risk factors of congenital heart disease of perinatal infants. Eating vegetables and fruits can reduce the risk of congenital heart disease, strengthen the health education of pregnant women, strengthen the nutritional supplement of pregnant women, keep the mood comfortable during pregnancy, avoid infection, and reduce the risk of congenital heart disease in perinatal infants.
