音乐治疗对早产儿脑电活动及行为神经发育的作用

【摘要】:目的:探讨音乐治疗(莫扎特音乐和五行音乐)对早产儿脑电活动和行为神经发育的作用。方法:使用前瞻性随机对照试验方法,将2016年02月至2017年01月在广州中医药大学附属南海妇产儿童医院新生儿科住院的胎龄在34-36周、无神经系统疾病的早产儿纳入研究。经过样本量估算和随机分配,共纳入90例受试者,莫扎特音乐组30例,五行音乐组30例,另有用于对照观察的空白对照组30例。莫扎特音乐组和五行音乐组在生后第3天开始每天进行2次音乐治疗,每次时长为10分钟,音量控制在40dB,莫扎特音乐组的音乐素材为莫扎特钢琴奏鸣曲,五行音乐组为羽音,空白对照组不予音乐治疗。比较音乐治疗开始前和音乐治疗过程中原始脑电图(EEG)脑电频谱的差异。比较生后第3天和生后第14天振幅整合脑电图(aEEG)的背景活动连续性、振幅下边界和带宽的差异。比较生后第3天和生后第14天行为神经评分(NBNA)的差异。受试者入组和出院前均行快速听觉脑干诱发电位检查,以了解音乐治疗的安全性。结果:1、与音乐治疗前比较,音乐治疗过程中的脑电频谱有显著变化,其中莫扎特音乐组以α频段(P㩳0.01)、β频段(P㩳0.05)的增多为主,五行音乐组以θ频段的增多为主(P㩳0.05)。2、生后第14天,莫扎特音乐组(P㩳0.05)和五行音乐组(P㩳0.01)的aEEG振幅下边界均高于空白对照组;莫扎特音乐组(P㩳0.05)和五行音乐组(P㩳0.01)的aEEG带宽均窄于空白对照组。3、生后第14天,莫扎特音乐组(P㩳0.01)和五行音乐组(P㩳0.05)的NBNA总分均高于空白对照组;莫扎特音乐组和五行音乐组的NBNA视听定向力加分均高于空白对照组(P均0.01)。4、莫扎特音乐组和五行音乐组受试者在音乐治疗期间未发现不良反应,出院前再次行快速听觉脑干诱发电位检查均未发现异常。结论:1、音乐治疗过程中可引起早产儿脑电频谱的变化。2、音乐治疗可促进早产儿脑电背景活动的成熟。3、音乐治疗可改善早产儿的行为神经发育。4、音乐治疗安全可靠。
[Abstract]:Objective: to investigate the effects of music therapy (Mozart music and five elements music) on brain electrical activity and behavioral nerve development in premature infants. From February 2016 to January 2017, the gestational age of preterm infants without nervous system diseases was 34-36 weeks in Nanhai Maternity and Childhood Hospital, Guangzhou University of traditional Chinese Medicine. The samples were estimated and randomly distributed. A total of 90 subjects were included: Mozart music group (30 cases), five elements music group (30 cases) and blank control group (30 cases). Mozart music group and Wuxing music group began to receive music therapy twice a day on the third day after birth. Each time is 10 minutes, volume control is 40 dB, Mozart group music material for Mozart piano sonata, five elements of music group for plume, Blank control group was not treated with music. The difference of EEG spectrum before and during music therapy was compared. The continuity of background activity of amplitude integrated EEG (EEGE) was compared on the 3rd day after birth and on the 14th day after birth. The differences of amplitude boundary and bandwidth were compared between postnatal day 3 and postnatal day 14. The subjects underwent rapid auditory brainstem evoked potential (BAEP) examination before and after admission. Results: 1, compared with before music therapy, there were significant changes in EEG spectrum during music therapy, especially in Mozart music group. 0.01g, 尾 band P? The total scores of NBNA in Mozart group and Wuxing music group were higher than those in blank control group (P < 0.01), and no adverse reactions were found in Mozart music group and Wuxing music group during music therapy, and the scores of NBNA in Mozart music group and Wuxing music group were higher than those in blank control group (P < 0.01). No abnormality was found in the rapid auditory brainstem evoked potential examination before discharge. Conclusion: 1: 1, music therapy can cause the changes of EEG spectrum of premature infants. 2. Music therapy can promote the maturation of EEG background activity of premature infants. Music therapy can improve behavioral neurodevelopment of preterm infants, and music therapy is safe and reliable.
