缺血修饰蛋白与肺炎合并心力衰竭新生儿心肌损伤程度及预后的相关性

【摘要】：目的探讨缺血修饰蛋白(IMA)对肺炎合并心力衰竭新生儿心肌损伤程度及预后的意义。方法选取肺炎、肺炎合并心力衰竭及正常新生儿各60例,检测其血清IMA、肌钙蛋白I(c Tn I)和肌酸激酶同工酶(CKMB)水平并进行比较。通过超声检测计算3组新生儿左心室心肌做功指数(Tei指数)并进行比较。分析肺炎合并心力衰竭组新生儿IMA与c Tn I、CK-MB及左心室Tei指数的相关性。随访肺炎合并心力衰竭组新生儿,记录该组患儿转归情况,分析IMA水平与预后的关系。结果 3组患儿的IMA、c Tn I和CK-MB差异有统计学意义(P0.05),肺炎合并心力衰竭组患儿IMA为(96.84±10.49)U/m L,c Tn I为(1.40±0.30)μg/L,CK-MB为(69.66±13.2)U/L,上述3种指标均显著高于其余两组(P0.05)。肺炎合并心力衰竭组左心室Tei指数为0.43±0.12,显著高于其余两组(P0.05)。肺炎合并心力衰竭组新生儿IMA与c Tn I、CK-MB及左心室Tei指数均呈显著正相关(r=0.81、0.73、0.69,P0.05)。肺炎合并心力衰竭组高IMA水平转归情况包括心律失常及死亡。结论缺血修饰蛋白可作为肺炎合并心力衰竭新生儿心肌损害程度及预后判断的有效指标。
[Abstract]:Objective to investigate the significance of ischemia modified protein (IMA) in myocardial injury and prognosis of neonates with pneumonia and heart failure. Methods 60 cases of pneumonia complicated with heart failure and 60 cases of normal neonates were selected. The levels of serum IMA, cardiac troponin I (TnI) and creatine kinase isoenzyme (CKMB) were measured and compared. The correlation between IMA and CK-MB and Tei index of left ventricle in neonates of failure group. The newborns in pneumonia complicated with heart failure group were followed up. The outcome of the group was recorded, Results the difference of IMAc TnI and CK-MB was statistically significant among the three groups, and the IMA of pneumonia with heart failure was 96.84 卤10.49 渭 g / m TnI (1.40 卤0.30) 渭 g / L + CK-MB = 69.66 卤13.2 渭 g / L, respectively. These three indexes were significantly higher than those of the other two groups. The Tei index of left ventricle in pneumonia with heart failure group was 0.43 卤0.12, which was significantly higher than that in the other two groups. There was a significant positive correlation between IMA and c TNI CK-MB and left ventricular Tei index in pneumonia with heart failure group. Conclusion Ischemic modified protein can be used as an effective indicator of myocardial damage and prognosis in neonates with pneumonia and heart failure.
