氯己定醇联合含碘手术薄膜对患儿行骨科手术切口感染的影响

【摘要】:目的探究氯己定醇联合含碘手术薄膜对儿童行骨科手术切口感染的影响。方法选取2015年4月-2016年5月于医院接受骨科手术的460例患儿为研究对象,按照随机数表法将所有患儿平均分为两组,每组230例,其中对照组患儿使用含碘消毒液联合含碘手术薄膜进行骨科手术,观察组患儿使用氯己定醇联合含碘手术薄膜进行骨科手术,比较两组患儿术后切口感染率、手术效果、揭开手术薄膜后细菌的阳性率;对发生感染的患儿进行病原菌培养与鉴定,分析病原菌的分布。结果观察组6例患儿发生切口感染,感染率为2.61%,观察组患儿感染人数以及感染率均低于对照组21例(9.13%),差异有统计学意义(χ~2=4.330,P0.05);观察组患者手术切口薄膜起边率以及手术愈合时间均低于对照组患者,甲级愈合率高于对照组患者,观察组患者揭开手术薄膜后细菌的阳性率低于对照组患者,差异有统计学意义(χ~2=4.116,P0.05);感染患儿临床送检标本共检测出病原菌67株,其中革兰阴性菌32株、革兰阳性菌27株、真菌6株、其他菌种2株。结论行骨科手术患儿切口感染的主要病原菌是革兰阴性菌,采用氯己定醇联合含碘手术薄膜能够有效降低手术患儿术后的感染率,提高术后恢复效果,对于提高患者的生活质量具有重要作用。
[Abstract]:Objective to investigate the effect of chlorhexidine combined with iodized film on incision infection in children undergoing orthopedic surgery.Methods A total of 460 children undergoing orthopedic surgery from April 2015 to May 2016 were randomly divided into two groups (230 cases in each group).The patients in the control group were treated with iodine disinfectant combined with iodized surgical film for orthopedic operation, and the patients in the observation group were treated with chlorhexidine and iodized film for orthopedic surgery. The infection rate of incision and the effect of operation were compared between the two groups.The positive rate of bacteria was detected and the distribution of pathogenic bacteria was analyzed by culture and identification of pathogenic bacteria in children with infection.Results incisional infection occurred in 6 children in the observation group.The infection rate was 2.61. The infection number and infection rate in the observation group were lower than that in the control group (21 cases), the difference was statistically significant (蠂 ~ 2 ~ (2) 4.330) P 0.05, the rate of the operation incision membrane and the operation healing time in the observation group were lower than those in the control group.The rate of grade A healing was higher than that of the control group, and the positive rate of bacteria in the observation group was lower than that in the control group (蠂 2 + 4.116% P 0.05), and 67 strains of pathogenic bacteria were detected in the clinical samples of the infected children, and the positive rate of bacteria in the observation group was significantly lower than that in the control group.There were 32 Gram-negative bacteria, 27 Gram-positive bacteria, 6 fungi and 2 other strains.Conclusion Gram-negative bacteria is the main pathogen of incision infection in children undergoing orthopedic surgery. The use of chlorhexidine alcohol combined with iodized film can effectively reduce the postoperative infection rate and improve the postoperative recovery in children with orthopedic surgery.It plays an important role in improving the quality of life of patients.
