101例婴儿斜头异常的临床分析

【摘要】:目的探讨引起婴儿斜头异常的可能原因,以及姿势矫正训练对斜头异常的改善效果。方法选择诊断为斜头异常的患儿101例进行研究,按照诊断斜头异常时的年龄分为3组:1~4月龄组(n=31)、5~8月龄组(n=40)和9~12月龄组(n=30)。对3组患儿斜头异常的可能原因进行调查分析,并对3组患儿给予姿势矫正训练前后头颅不对称指数(CVAI)进行比较。结果 101例患儿中,89例(88.1%)患儿为仰卧位固定睡姿,3组间此睡姿率比较差异无统计学意义。1~4月龄组患儿早产出生率、不良围产因素发生率及先天性肌性斜颈的发生率高于5~8月龄组和9~12月龄组,差异有统计学意义。3组患儿姿势矫正训练3个月后CVAI值均较训练前显著下降(P0.001)。1~4月龄组给予姿势矫正训练前后CVAI差值显著大于5~8月龄组和9~12月龄组(P0.001)。结论仰卧位固定睡姿可能与婴儿斜头异常的发生有关;不良围产因素、早产出生及先天性肌性斜颈作为婴儿斜头异常的可能原因更常见于婴儿早期;姿势矫正训练对婴儿斜头异常改善有意义,特别是对婴儿早期的斜头异常的改善效果更明显。
[Abstract]:Objective to explore the possible causes of the abnormal oblique head in infants and the effect of posture correction training on the abnormal oblique head. Methods 101 cases of children with abnormal oblique head were selected and studied. According to the age of diagnosis of diagonal head abnormality, the patients were divided into 3 groups: 1 / 1 / 4 month old group (n = 31) and 9 ~ 12 months old group (n = 30). The possible causes of the abnormal head of oblique head in the three groups were investigated and analyzed. The cranial asymmetry index (CVAII) before and after postural correction training was compared among the 3 groups. Results there was no significant difference in the sleeping position rate between the three groups for supine position fixed sleeping position in 89 out of 101 children (88. 1). There was no significant difference in the sleeping position rate among the 3 groups at the age of 4 months. Preterm birth rate, The incidence of adverse perinatal factors and congenital muscular torticollis was higher than that of 5-month group and 9-month group. The difference of CVAI between postural correction group and postural correction training group after 3 months training was significantly lower than that before and after postural correction training. The difference of CVAI between postural correction group and postural correction training group was significantly higher than that in 5-month group and 91-month group. Conclusion supine position fixation is significantly higher than that of postural orthodontic group before and after postural orthodontic training. Conclusion supine position fixation is significantly higher than that of postural orthodontic training. The determination of sleeping position may be related to the occurrence of abnormal oblique head in infants. Adverse perinatal factors, preterm birth and congenital muscular torticollis are more common in the early stage of infants. In particular, the improvement of early infants with abnormal oblique head is more obvious.
