托吡酯、丙戊酸钠对癫痫患儿体质量及糖代谢指标的影响

【摘要】：目的观察托吡酯(TPM)与丙戊酸钠(VPA)对癫痫患儿体质量及糖代谢指标的影响。方法选择首次诊断癫痫TPM或VPA单药治疗的患儿各30例,分别于治疗前、治疗3个月和6个月进行体质量、空腹血糖、空腹胰岛素、胰岛素抵抗(IR)指数、瘦素、脂联素、瘦素／脂联素(L/A)测定,行TPM、VPA治疗前后自身对照研究。结果TPM组：治疗3个月的患儿空腹胰岛素水平、IR指数、瘦素水平、L/A较治疗前均明显降低(均P0.01),体质量和脂联素水平变化不明显(均P0.05)；治疗6个月的患儿空腹胰岛素水平、IR指数、瘦素水平、L/A较治疗3个月时亦明显降低(均P0.01),体质量较治疗前、治疗3个月时均明显增高(P0.05),但脂联素水平较治疗前明显增高(P0.01): TPM治疗前后空腹血糖水平无显著区别(P0.05)。 VPA组：治疗3个月的患儿体质量、空腹胰岛素水平、IR指数、瘦素水平、L/A较治疗前均明显升高(均P0.05)但脂联素水平较治疗前明显降低(P0.01)；VPA治疗6个月的患儿体质量、瘦素水平、L/A较治疗3个月时亦明显升高(均P0.05)而脂联素水平较治疗3个月时下降更明显(P0.01),空腹胰岛素水平、IR指数较治疗前、治疗3个月时均明显降低(均P 0.01); VPA治疗前后空腹血糖水平无显著区别(P0.05)。结论(1)TPM治疗后使癫痫患儿体质量增长缓慢,而不是真正的体质量下降。 (2)VPA治疗后使癫痫患儿体质量增加。 (3) TPM、VPA治疗后可引起糖代谢指标的变化。(4) TPM、PA治疗后对血糖水平无显著影响。
[Abstract]:Objective to observe the effects of topiramate (TPM) and sodium valproate (VPA) on body mass and glucose metabolism in epileptic children.Methods 30 children with epilepsy treated with TPM or VPA for the first time were given body mass, fasting blood glucose, fasting insulin, insulin resistance index, leptin and adiponectin before treatment for 3 and 6 months, respectively.Leptin / adiponectin L / A was measured and self-controlled study was performed before and after treatment with TPM VPA.Results in TPM group, the fasting insulin level and IR index, leptin level and L / A were significantly decreased after 3 months of treatment (all P 0.01), the body weight and adiponectin levels were not changed significantly (all P 0.05), and the fasting insulin level of 6 months after treatment was significantly lower than that of before treatment (P 0.05), and no significant changes were found in the body mass and adiponectin levels (P 0.05).Leptin level was significantly lower than that at 3 months after treatment (all P0.01A, body weight was higher than that before treatment, P0.05A was significantly higher than that before treatment, but adiponectin level was significantly higher than that before and after treatment. There was no significant difference in fasting blood glucose level before and after treatment with TPM.In VPA group, the body mass, fasting insulin level, IR index and leptin level were significantly higher than those before treatment (all P0.05), but adiponectin level was significantly lower than that before treatment.The levels of leptin, L / A and adiponectin were significantly higher than those at 3 months after treatment (all P 0.05), and the levels of adiponectin were significantly lower than those at 3 months after treatment (P 0.01), and the fasting insulin level and IR index were higher than those before treatment.After 3 months of treatment, the level of fasting blood glucose decreased significantly (all P 0.01), and there was no significant difference in fasting blood glucose level before and after treatment with VPA (P 0.05).Conclusion the body mass of epileptic children increased slowly but not the real body mass decreased after TPM treatment.After treatment with VPA, the body mass of epileptic children was increased.(3) TPMN VPA could cause the change of glucose metabolism index after treatment. 4) TPMN PA had no significant effect on blood glucose level after treatment.
