喜炎平注射液治疗小儿支气管炎有效性、安全性、经济性系统评价

【摘要】:目的:本文采用系统评价方法对喜炎平注射液治疗小儿支气管炎有效性、安全性进行分析,在meta分析基础上采用成本-效果、最小成本法评价喜炎平注射液的经济性。为临床合理用药提供科学依据。方法:循证医学系统评价法。以喜炎平注射液、喜炎平注射液+小儿支气管炎、喜炎平注射液+常规治疗为主题词,检索中国学术期刊全文数据库(CNKI)、中文科技期刊全文数据库(VIP)、万方数据库(WangfangDate)、中国生物医学文献数据库(CBM)、及PubMed、EMBASE、MEDLINE等数据库。采用Meta分析方法,对喜炎平注射液治疗小儿支气管炎的有效性和安全性评价。收集315网站有关喜炎平注射液的价格信息,在meta分析基础上采用成本-效果分析,最小成本法,评价喜炎平注射液治疗小儿支气管炎的经济性。结果:有效性、安全性:喜炎平注射液与炎琥宁注射液治疗小儿急性支气管炎总有效率比较:使用喜炎平注射液组总有效率与使用炎琥宁注射液组比较差异有统计学意义(RR= 1.34,95%CI:1.26,1.43,P0.00001),不良反应发生率比较:使用喜炎平注射液组与使用炎琥宁注射液组比较,差异有统计学意义(RR=0.29,95%CI:0.15,0.55,P=0.0002);喜炎平注射液+常规疗法与常规疗法治疗小儿毛细支气管炎总有效率比较:使用喜炎平注射液+常规疗法组与仅用常规疗法组比较,总有效率差异有统计学意义(OR=10.34,95%CI:4.97,20.56,P0.00001);喜炎平注射液与利巴韦林注射液治疗小儿毛细支气管炎总有效率比较:使用喜炎平注射液组与使用利巴韦林注射液组比较,差异有统计学意义(OR=4.72,95%CI:3.11,7.18,P0.00001)。经济性:喜炎平注射液与炎琥宁注射液治疗小儿急性支气管炎成本-效果分析比较:使用喜炎平组C/E=123.93,使用炎琥宁组C/E=39.47,增量成本效果分析,ICER=371.98,敏感性分析,ICER= 420.75;喜炎平注射液+常规疗法与常规疗法治疗小儿毛细支气管炎成本-效果分析:使用喜炎平注射液+综合疗法组C/E(不含误工费)=179.63,C/E(含误工费)=179.63;仅使用综合疗法组C/E(不含误工费)=1220.31,C/E(含误工费)=2037.11。敏感性分析,使用喜炎平注射液+综合疗法组C/E(不含误工费)=197.59,C/E(含误工费)=197.59,仅使用综合疗法组C/E(不含误工费)=1220.31,C/E(含误工费)=2037.11;喜炎平注射液与利巴韦林注射液治疗小儿毛细支气管炎成本-效果分析:使用喜炎平注射液组C/E(不含误工费)=238.57,C/E(含误工费)=238.57,使用利巴韦林注射液组C/E(不含误工费)=1231.7 C/E(含误工费)=2053.68,敏感性分析,使用喜炎平注射液组C/E(不含误工费)=262.42,C/E(含误工费)=262.42,使用利巴韦林注射液组C/E(不含误工费)=1231.70,C/E(含误工费)=2053.68结论:喜炎平注射液用于治疗小儿支气管炎安全性较高,有效性和经济性具有一定的优势。尤其是在治疗小儿毛细支气管炎方面,在产生较高的总有效率的同时只需要较低的治疗成本,所以是具有优势的治疗方案。具有较好的临床研究和应用前景。但本系统评价所纳入的研究文献质量不高,增加了本次系统评价结论产生偏倚的风险。
[Abstract]:Objective: to analyze the efficacy and safety of the yanyanping injection in the treatment of bronchitis in children by using the method of systematic evaluation. On the basis of meta analysis, the cost effectiveness and minimum cost method was used to evaluate the economy of the yanyanping injection. Ejection liquid, yanyanping injection + bronchitis, yanyanping injection + routine treatment as the subject words, retrieval of Chinese academic journal full text database (CNKI), Chinese scientific journal full text database (VIP), Wanfang database (WangfangDate), Chinese biomedical literature database (CBM), and PubMed, EMBASE, MEDLINE and other databases. Using Meta analysis party Method, evaluate the efficacy and safety of Xi Yanping injection in treating children bronchitis. Collect the price information about Xi Yanping injection on 315 websites, use the cost effect analysis and the least cost method on the basis of meta analysis, evaluate the economy of Xi Yanping injection in the treatment of children bronchitis. Results: effectiveness and safety: Xi Yanping note The total effective rate of injection and Potassium Sodium Dehydroandroandrographolide Succinate Injection in the treatment of acute bronchitis in children: the total effective rate of using the yanyanping injection group was statistically significant compared with that of the Potassium Sodium Dehydroandroandrographolide Succinate Injection group (RR= 1.34,95%CI:1.26,1.43, P0.00001), and the incidence of adverse reactions was compared: the ratio of the Yanping injection group to the use of the Potassium Sodium Dehydroandroandrographolide Succinate Injection group was compared with that of the Potassium Sodium Dehydroandroandrographolide Succinate Injection group. The difference was statistically significant (RR=0.29,95%CI:0.15,0.55, P=0.0002); the total effective rate of Xi Yanping injection + conventional therapy and conventional therapy in the treatment of children's bronchiolitis was compared: the total effective difference was statistically significant (OR=10.34,95%CI:4.97,20.56, P0.0000) with conventional therapy group and conventional therapy group. 1): comparison of the total efficiency of Xi Yanping injection and Ribavirin Injection in the treatment of bronchiolitis in children: the difference was statistically significant (OR=4.72,95%CI:3.11,7.18, P0.00001) with the use of Xi Yanping injection group and the use of Ribavirin Injection group. Ejection group C/E (excluding misunderstanding fee) =262.42, C/E (including misunderstanding fee) =262.42, use of Ribavirin Injection group C/E (excluding misunderstanding fee) =1231.70, C/E (including misunderstanding fee) =2053.68 conclusion: the yanyanping injection is used to treat children bronchitis with high safety, effectiveness and economy, especially in the treatment of bronchioles in children. In the case of high total efficiency, it only requires lower cost of treatment, so it is an advantageous treatment. It has good clinical research and application prospects. However, the quality of the literature included in this system evaluation is not high, which increases the risk of bias in the conclusion of this system evaluation.
