口服葡萄糖对新生儿操作性疼痛干预效果的临床研究

【摘要】：目的：通过探讨2种浓度葡萄糖对新生儿桡动脉采血所致疼痛的影响,为新生儿疼痛护理提供科学依据。方法：采用随机分组原则,将90例新生儿随机分成3组：A、B、C组,每组各30例,于桡动脉采血针刺前2min,A组新生儿予口服浓度为10%的葡萄糖2ml,B组新生儿予口服浓度为25%的葡萄糖2ml,C组新生儿予口服温白开水2ml。采用新生儿面部编码系统(NFCS)、新生儿疼痛量表(NIPS)、心率上升幅度和啼哭例数作为疼痛观察指标。采用SPSS13.0软件包对各组资料进行统计分析,统计结果以p0.05为差异有统计学意义。结果： 1、组间比较,三组新生儿疼痛评分(包括针刺时的NIPS评分、结束时的NFCS评分和NIPS评分)分值、心率上升幅度以及啼哭例数比较差异均有统计学意义(p0.05),两两比较结果：B组明显低于A、C两组,而A、C两组间比较差异无显著统计学意义(p0.05)。 2、同组比较,各组新生儿针刺时和结束时疼痛评分(包括NFCS评分和NIPS评分)分值比较差异均有统计学意义(p0.05),针刺时疼痛分值均明显高于结束时疼痛分值。结论： 1、新生儿接受疼痛刺激前2min,予口服25%葡萄糖2ml可明显降低疼痛评分分值、心率上升幅度和减少啼哭,而10%葡萄糖2ml镇痛作用不明显。 2、口服25%葡萄糖用于缓解新生儿操作性疼痛简单有效,安全可行。
[Abstract]:Objective: to explore the effect of two concentrations of glucose on the pain caused by radial artery blood sampling in neonates, and to provide scientific basis for the nursing of neonatal pain. Methods: 90 newborns were randomly divided into 3 groups with 30 cases in each group. The newborns in group A were given orally 10% glucose 2 ml / L group B and 25% glucose 2 ml group C newborns were given 2 ml of warm boiled water 2 ml before radial artery blood sampling acupuncture 2 mins before acupuncture. 2 min before acupuncture, the newborns in group A were given oral administration of glucose 2 ml of 10% and group B was given oral concentration of 25% glucose 2 ml. The neonatal facial coding system (NFCSA), neonatal pain scale (NIPSV), heart rate rise (HR) and number of crying cases were used to observe the pain. SPSS13.0 software package was used to analyze the data of each group, and the statistical results were statistically significant with p0. 05 as the difference. Results: 1. The scores of neonatal pain score (including NIPS score at acupuncture, NFCS score and NIPS score at the end of acupuncture) were compared among the three groups. There were significant differences in the heart rate and the number of crying cases. The results showed that the ratio of heart rate in group B was significantly lower than that in group A and C, but there was no significant difference between group A and group C (p 0.05). 2Compared with the same group, the scores of pain score (including NFCS score and NIPS score) at the time of acupuncture and at the end of acupuncture in each group were significantly different (P 0.05), and the scores of pain at acupuncture were significantly higher than those at the end of acupuncture. Conclusion: 1. Two minutes before pain stimulation, 25% glucose 2ml could significantly decrease the score of pain, increase heart rate and reduce crying, but 10% glucose 2ml had no obvious analgesic effect. 2, oral administration of 25% glucose is simple, effective and safe to relieve neonatal operative pain.
