全氟化合物对儿童肺功能影响的病例对照研究

【摘要】:目的探讨全氟化合物对哮喘儿童和一般儿童肺功能影响的性别差异,为采取有效措施保护儿童心肺功能提供参考。方法从台湾大学医院就诊的哮喘儿童选取132名,从台北地区7所公立中学随机选取168名一般健康儿童。采用Logistic回归和多重线性回归的方法分析血清全氟辛烷磺酸(perfluorooctanesulfonate,PFOS)和全氟辛酸(perfluorooctanoic acid,PFOA)水平对儿童肺功能的影响。结果儿童哮喘的患病风险随着全氟化合物(perfluoroalkyl and polyfluoroalkyl substances,PFAAS)浓度的增加而呈现增高趋势,PFOS与PFOA暴露对儿童哮喘患病风险的优势比分别为1.32(95%CI=1.02~1.71)和2.80(95%CI=1.87~4.20)。PFOS和PFOA暴露水平与哮喘男童的肺功能水平(FVC,FEV_1和FEF_(25%~75%))呈负相关,男孩血清中PFOS和PFOA水平每增加1μg/L时,FEV_1分别下降-0.062(95%CI=-0.106~-0.018)和-0.099(95%CI=-0.197~-0.001)L。结论 PFAAs的暴露可以降低儿童肺功能水平,并且男童相较于女童更易感。
[Abstract]:Objective to explore the effect of perfluorocarbons on lung function in asthmatic children and general children, and to provide reference for effective measures to protect children's cardiopulmonary function. Methods 132 asthmatic children were selected from Taiwan University Hospital and 168 healthy children were randomly selected from 7 public middle schools in Taipei area. The effects of serum perfluorooctanesulfonate (PFOS) and perfluorooctanoic acid (PFOAA) levels on children's lung function were analyzed by Logistic regression and multiple linear regression. Results the risk of asthma in children increased with the increase of perfluoroalkyl and polyfluoroalkyl substancesPFAAS. the odds ratio between PFOS and PFOA exposure to childhood asthma risk was 1.3295 CI 1.02C 1.71), and the exposure levels of 2.80(95%CI=1.87~4.20).PFOS and PFOA were higher than those of asthmatic boys. There was a negative correlation between the level of pulmonary function and the FEFC number of FEV1. When the serum levels of PFOS and PFOA increased by 1 渭 g / L in boys, FEV _ 1 decreased -0.062 ~ (95) CI ~ (-0.106) ~ (-0.018) and -0.099 ~ 95% ~ (-0.197) ~ (-0.001) L ~ (-1) respectively. Conclusion exposure to PFAAs can reduce pulmonary function in children, and boys are more susceptible than girls.
