调节性B细胞在儿童哮喘发病中的初步研究

【摘要】:目的:探讨调节性B细胞(Regulatory B cells,Breg)在儿童哮喘发病中可能的作用。方法:选取急性期哮喘患儿23例,平均年龄6.58±2.38岁,男女比例:2.83:1;同年龄健康儿童21例,平均年龄5.43±1.91岁,男女比例2.00:1;(1)采用流式细胞术检测外周血Treg、Th1、Th2、Th17、Breg比例及CD80、CD86、CD1d表达水平;(2)实时荧光定量PCR检测IFN-γ、T-bet、GATA3、IL-17A、IL-17F、RORγt、IL-10、TGF-βm RNA表达;(3)采用ELISA检测血清IL-10浓度;结果:(1)急性期哮喘患儿外周血中CD24hi CD27~+Breg细胞比例明显低于正常对照组,共刺激分子CD80、CD86表达水平低于正常对照组(P0.05),而CD1d的表达水平有下降趋势(P0.05);(2)急性期哮喘患儿的CD4~+CD25~+Foxp3~+Treg比例低于正常对照组(P0.05);(3)急性期哮喘患儿外周血清的IL-10浓度低于正常对照组,有统计学意义(P0.05);(4)急性期哮喘患儿外周血Th1细胞比例降低,而Th2及Th17细胞比例增高,均有统计学意义(P0.05);(5)荧光定量PCR检测观察到急性期哮喘患儿中CD19~+B细胞中IL-10的表达无明显差异,而TGF-β的表达水平是升高的有统计学意义(P0.05);(6)急性期哮喘患儿CD4~+T细胞中T-bet、IFNG、RORγt、IL-17A、IL-17F表达上调,均具有统计学意义(P0.05);GATA3表达未见显著性差异(P0.05)。结论:CD24hi CD27~+Breg比例下降及分泌IL-10功能减弱可能是导致急性期哮喘患儿CD4~+T细胞诱导分化异常的重要因素,共刺激分子CD80、CD86表达下调可能也影响了急性期哮喘患儿CD24hiCD27~+Breg的免疫调节功能。
[Abstract]:Objective: to investigate the possible role of regulatory B cell Bregin in the pathogenesis of asthma in children. Methods: 23 children with acute asthma (mean age: 6.58 卤2.38 years) and 21 healthy children of the same age (mean age: 5.43 卤1.91 years) were selected. Ratio of male to female (2.00: 1 / 1). The ratio of Treg-Th1Th2T / Th17G and CD80 / CD86 / CD1d in peripheral blood were detected by flow cytometry (FCM) and the expression of TGF- 尾 m RNA in peripheral blood was detected by flow cytometry (FCM). Real time quantitative PCR was used to detect the expression of IL-17FOR 纬 IL-10 TGF- 尾 m RNA by real-time quantitative PCR. The serum IL-10 concentration was detected by ELISA. Results the percentage of CD24hi CD27 ~ Breg cells in peripheral blood of children with acute asthma was significantly lower than that of normal controls. The expression level of CD80T CD86 was lower than that of the normal control group (P0.05A) and the expression of CD1d was lower than that of the control group (P0.05-Foxp3- Treg was lower than that of the control group (P0.05FXP3-) the level of IL-10 in the peripheral blood of the children with acute asthma was lower than that of the normal control group (P < 0.05), but the level of IL-10 in the peripheral blood of the children with acute asthma was lower than that of the control group (P < 0.05). The proportion of Th1 cells in peripheral blood of children with acute asthma was decreased, while the proportion of Th2 and Th17 cells was increased. There was no significant difference in the expression of IL-10 in CD19 ~ B cells in children with acute asthma by fluorescence quantitative PCR. The expression of TGF- 尾 in CD4 ~ T cells of children with acute asthma was significantly increased (P 0.05). The expression of TGF- 尾 was up-regulated in CD4 ~ T cells of children with acute asthma. There was no significant difference in the expression of TGF- 尾 between the two groups (P 0.05). Conclusion the decreased proportion of CD24hi CD27 ~ Breg and the decrease of IL-10 secretion may be important factors in inducing abnormal differentiation of CD4 ~ T cells in children with acute asthma. The down-regulation of CD80 and CD86 may also affect the immunomodulatory function of CD24hiCD27 ~ Breg in children with acute asthma.
