脓毒症血流感染患儿血清C-反应蛋白、降钙素原水平变化及临床诊断分析

【摘要】：目的探讨脓毒症血流感染患儿血清C-反应蛋白(CRP)、降钙素原(PCT)水平变化及临床诊断意义。方法选择2010年1月-2016年6月医院接诊60例脓毒症血流感染患儿为脓毒症感染组,并选择同期接诊60例其余部位感染的患儿为对照组,比较两组患儿血清CRP、PCT水平,PCT的测定使用免疫荧光双抗体夹心法,CRP的测定使用胶体金法。结果脓毒症血流感染患儿血清CRP(138.93±29.31)mg/L、PCT(13.21±1.87)ng/ml水平明显高于对照组(47.34±9.84)mg/L、(0.31±0.05)ng/ml,差异有统计学意义(P0.05);革兰阳性菌血流感染脓毒血症患儿CRP(73.12±17.84)mg/L、PCT(2.31±0.44)ng/ml水平明显低于革兰阴性菌感染患儿(116.72±29.79)mg/L、(14.82±1.37)ng/ml,差异有统计学意义(P0.05);在革兰阳性菌、革兰阴性菌血流感染脓毒血症患儿、其他所有血流感染脓毒血症患儿均和CRP、PCT之间呈正相关(r=0.631,0.682,0.564;P0.05)。结论脓毒症血流感染患儿CRP、PCT比其余部位感染的患儿表达水平更高,且革兰阴性菌感染患儿CRP、PCT水平高于革兰阳性菌感染患儿,在脓毒症血流感染患儿中联合检测血清CRP、PCT具有较高的诊断价值,可为临床诊断及治疗提供可靠依据,值得应用推广。
[Abstract]:Objective to investigate the changes of serum C-reactive protein (CRP) and procalcitonin (procalcitonin) levels in children with septic blood stream infection and their clinical diagnostic significance. Methods from January 2010 to June 2016, 60 children with septic blood flow infection were selected as sepsis infection group, and 60 patients with other site infection were selected as control group. The serum levels of CRPnc-PCT and PCT were measured by immunofluorescence double antibody sandwich method and the level of CRP was determined by colloidal gold method. Results the levels of serum CRP(138.93 卤29.31 mg / L PCT 13.21 卤1.87)ng/ml in children with septic blood flow infection were significantly higher than those in the control group (47.34 卤9.84 mg / ml, P < 0.05), and the levels of CRP(73.12 卤17.84 mg / L CRP(73.12 / L ~ (2.31 卤0.44)ng/ml) in children with septic sepsis were significantly lower than those in patients with Gram-negative bacterial infection (116.72 卤29.79 mg / L, 14.82 卤1.37 ng / ml, P < 0.05). In Gram-positive bacteria, There was a positive correlation between the blood flow infection of Gram-negative bacteria and CRPnPCT in children with sepsis of all other blood flow infections (P 0.05). Conclusion the expression of CRPnPCT in children with septic blood stream infection is higher than that in other infected children, and the level of CRPnPCT in Gram-negative bacteria infection is higher than that in Gram-positive infection. The combined detection of serum CRPnPCT in children with septic blood stream infection is of high diagnostic value and can provide reliable basis for clinical diagnosis and treatment.
