应变及应变率成像评估蒽环类药物化疗后儿童左室心肌运动的研究

【摘要】:目的探讨二维斑点追踪成像技术(2D-STI)在早期评估蒽环类化疗药对血液系统恶性肿瘤疾病儿童左室心肌损害的价值。方法分析120例接受蒽环类化疗药物治疗的血液系统恶性肿瘤疾病儿童(病例组)及89例正常儿童(对照组)的超声心动图,应用斑点追踪软件分析左心室心尖长轴切面的动态图像,获取并分析左心室收缩期心肌纵向应变及应变率。结果病例组120例儿童心肌标志物检测及常规超声心动图EF、FS检查结果均正常,18节段应变和应变率值均小于正常对照组,差异有统计学意义(P0.05)。蒽环类化疗药单位体表面积的累积剂量越大,心室壁节段应变及应变率越小,差异有统计学意义(P0.05);蒽环类化疗药对心肌功能的影响与单位体表面积累积剂量正相关,相关性有统计学意义(P0.05);SrL与化疗药物累积剂量相关性强于SL(P0.05)。结论2D-STI技术可早期发现蒽环类化疗药物对血液系统恶性肿瘤患儿左心室长轴心肌收缩功能异常,对临床早期干预有指导价值。
[Abstract]:Objective to evaluate the value of 2D-STI in early assessment of left ventricular myocardial damage by anthracycline chemotherapy in children with hematological malignancies. Methods Echocardiography was performed in 120 children with hematological malignancies treated with anthracycline chemotherapeutic agents (case group) and 89 normal children (control group). The dynamic images of long axis section of left ventricular apex were analyzed by dot tracing software. The longitudinal strain and strain rate of left ventricular systolic myocardium were obtained and analyzed. Results Myocardial markers of 120 children and routine echocardiography EFF FS were significantly lower than those of the normal control group (P 0.05), and the strain and strain rate values of normal 18 segments were significantly lower than those of the normal control group (P < 0.05). The greater the cumulative dose per unit body surface area of anthracycline chemotherapeutic agents, the smaller the strain and strain rate of ventricular wall segment, the difference being statistically significant (P 0.05), the effect of anthracycline chemotherapeutic agents on myocardial function was positively correlated with cumulative dose per unit body surface area. The correlation between SRL and the cumulative dose of chemotherapeutic drugs was significantly higher than that of SLP 0.05. Conclusion 2D-STI technique can detect the abnormal systolic function of left ventricular long axis muscle in children with hematological malignancy, and it is valuable for early clinical intervention.
