早产儿与足月儿精细运动发育差异及影响因素的研究

【摘要】:目的了解早产儿与足月儿精细运动发育水平的差异及其影响因素。 方法采用Peabody运动发育量表第二版(Peabody DevelopmentalMotor Scales2即PDMS-2)对215例婴儿(早产儿97例、足月儿118例)进行精细运动发育水平测试,并对所得结果精细运动发育商(FMQ)进行统计分析。 结果1)精细运动发育的抓握原始分和视觉运动整合原始分均随月龄的增加而增加,不同年龄段FMQ(矫正前)、FMQ(矫正后)、FMQ(足月)不同,(F=25.135P<0.05;F=28.634P<0.05;F=12.151P<0.05);2)早产儿FMQ(矫正前)89.58±5.43显著小于足月儿FMQ(足月)93.90±6.65(t=5.142P<0.05);3)精细运动的抓握标准分低于视觉运动整合标准分,(d=-1.00±1.24,t=-11.881P<0.05);4)早产儿的抓握和视觉运动整合的标准分均落后于足月儿,(t=4.695P<0.05;t=4.385P<0.05);5)无论早产儿还是足月儿,,均有抓握相当月龄和视觉运动整合相当月龄小于婴儿月龄,差异有统计学意义(P<0.05);6)FMQ受到胎龄、出生体重、是否试管婴儿、进食能力训练、身高、体重多方面因素的影响。 结论婴儿期精细运动的发育随年龄的增加不断发展,6月-1岁期间,精细运动发育的速度逐渐减慢;早产儿精细运动发育落后于足月儿,且抓握能力发育较视觉运动整合能力发育落后;孕期加强保健延长胎龄增加出生体重、生后有意识通过进食能力训练加强动手能力的提高、保持适宜的生长发育对精细运动的发育极其重要。
[Abstract]:Objective to investigate the difference of fine motor development between preterm and term infants and its influencing factors. Methods the fine motor development level of 215 infants (97 preterm infants and 118 full-term infants) was measured by Peabody motor development scale (PDMS-2), and the results were statistically analyzed. Results 1) the grasping primordial score and visual motor integration score of fine motor development increased with the increase of age. FMQs of preterm infants (89.58 卤5.43 before correction) were significantly lower than those of full-term infants (93.90 卤6.65(t=5.142P < 0.05). The grasp standard scores of preterm infants were lower than those of visual motor integration standard scores (-1.00 卤1.24t- 11.881P < 0.05t- 11.881P < 0.05). The standard score of motor integration was lower than that of term infants (4.695 P < 0.05 P < 0.05 P < 0.05). There were significant differences in grasping age and visual motor integration between the two groups (P < 0.05). FMQ was affected by the factors of gestational age, birth weight, test-tube infant, feeding ability training, height and weight. Conclusion the development of fine motor in infancy develops continuously with the increase of age, and the rate of fine motor development slows down gradually during June 1, and the fine motor development of preterm infants lags behind that of full-term infants. The development of grip ability was backward than that of visual motor integration ability, and the health care during pregnancy prolonged the gestational age to increase birth weight, and consciously increased the practical ability through feeding ability training after birth. Maintaining proper growth and development is essential for the development of fine motion.
