早产儿肺功能检查的临床研究

【摘要】:目的通过监测早产儿生后早期和纠正胎龄满6个月的潮气呼吸肺功能数据,比较生长前后的肺功能发育状态,探讨早产出生婴儿肺功能发育的影响因素。方法选取60例于广西壮族自治区人民医院儿科住院的孕32-36+6周出生的早产儿作为研究对象。其中,中期早产儿30例;晚期早产儿30例。应用德国耶格Masterscreen肺功能仪检测,分别于早产儿出生10天内及纠正胎龄满6个月进行潮气呼吸肺功能检测。结果1.早产儿的RR为54.93±11.27(次)高于足月儿46.90±6.70(次),两组比较有统计学意义(P0.05);早产儿PTEF(38.60±7.49)、TEF75(36.03±8.45)、TEF50(36.10±7.18)、TEF25(27.05±6.57)低于足月儿PTEF(60.67±13.32)、TEF75(55.23±13.25)、TEF50(58.87±13.27)、TEF25(48.60±10.60),两组比较差异均有统计学意义(p0.05);早产儿TPTEF/TE(31.76±9.74)、VPEF/VE(31.61±7.64)低于足月儿TPTEF/TE(40.62±7.05)、VPEF/VE(38.32±5.88),但差异无统计学意义(p0.05)。2.中期早产儿与晚期早产儿两组的TPTEF/TE、VPEF/VE、VT、VT/kg、RR、PTEF、TEF75、TEF50、TEF25比较差异均没有统计学意义(p0.05)。3.中期早产儿纠正胎龄6个月的TPTEF/TE、VPEF/VE、VT、PTEF、TEF75、TEF50和TEF25均明显高于出生10天内,两组比较均有统计学意义(P0.01);中期早产儿出生10天内的RR为52.45±12.22(次)高于纠正胎龄6个月35.55±6.74(次),两组比较有统计学意义(P0.01)。4.晚期早产儿纠正胎龄6个月的TPTEF/TE、VPEF/VE、VT、PTEF、TEF75、TEF50和TEF25均明显高于出生10天内,两组比较均有统计学意义(P0.01);晚期早产儿出生10天内的RR为51.60±10.19(次)高于纠正胎龄6个月36.47±8.10(次),两组比较有统计学意义(P0.01)。结论1.早产儿呼吸频率增快,呼气流速较低,气道阻力高。2.中期早产儿肺功能与晚期早产儿肺功能参数基本相当。3.早产儿潮气肺功能随年龄生长变化。4.潮气肺功能检测对早产儿肺发育的评估有临床价值。
[Abstract]:Objective to investigate the influencing factors of lung function development in premature infants by monitoring the lung function data of preterm infants at early postnatal stage and correcting 6 months of gestational age and comparing the development status of lung function before and after growth. Methods A total of 60 preterm infants with 32-36 6 weeks gestation in pediatrics of Guangxi Zhuang Autonomous region people's Hospital were selected as study objects. Among them, 30 cases were intermediate premature infants and 30 cases were late premature infants. The pulmonary function of preterm infants was measured by using the Jager Masterscreen pulmonary function instrument within 10 days of birth and 6 months of corrected gestational age.
