TLR9、MyD88及NF-κB在过敏性紫癜中的表达及临床意义

【摘要】:目的通过检测Toll样受体9(Toll like receptor 9,TLR9)、髓样分化因子88(myeloid different factory88,My D88)及核转录因子Kappa-B(nuclear factor kappa-B,NF-κB)m RNA在过敏性紫癜(Henoch-Schonlein purpura,HSP)患儿外周血单核细胞的表达,同时检测HSP患儿血浆细胞因子白介素6(interleukin-6,IL-6)、肿瘤坏死因子α(Tumor Necrosis Factor alpha,TNF-α)水平,探讨TLR9-My D88-NF-κB信号通路在HSP发病机制的作用,为治疗HSP寻找免疫上游靶点提供基础。方法选取2015年6月至2016年12月郑州大学第二附属医院儿科门诊及住院急性期HSP患儿40例作为研究对象,另选取同期在儿科门诊定期复查已痊愈HSP儿童35例作为正常对照组。采用ELISA法检测血浆IL-6、TNF-α水平,两组间比较采用独立样本t检验,相关性分析采用Pearson相关检验。结果1.HSP患儿外周血单核细胞TLR9、My D88及NF-κB m RNA表达均显著高于对照组(t=5.68,P0.01;t=5.42,P0.01;t=112.95,P0.01),差异有统计学意义;2.HSP组血浆IL-6、TNF-α水平较对照组水平显著增高(t=5.11,P0.01;t=9.26,P0.01),差别有统计学意义;3.HSP组中TLR9与My D88,TLR9与NF-κB,My D88与NF-κB,NF-κB与IL-6以及NF-κB与TNF-α均存在线性正相关关系(r=1,P0.01;r=0.98,P0.01;r=0.98,P0.01;r=0.98,P0.01;r=0.97,P0.01)。结论HSP患儿TLR9、My D88、NF-κB m RNA、IL-6、TNF-α水平均较对照组儿童显著升高。免疫上游因子TLR9、My D88、NF-κB对免疫下游细胞因子IL-6、TNF-α释放具有同步关联性。TLR9-My D88-NF-κB信号通路参与HSP发病,并在其中起重要作用。
[Abstract]:Objective to detect the expression of Toll like receptor 9 (Toll like receptor 9 TLR9, myeloid differentiation factor 88 (myeloid different factory88,My D88 and nuclear transcription factor Kappa-B (nuclear factor kappa-B,NF- 魏 B) m RNA in allergic purpura (Henoch-Schonlein purpura,). The levels of plasma cytokine interleukin-6 (interleukin-6,IL-6), tumor necrosis factor 伪 (Tumor Necrosis Factor alpha,TNF- 伪 (TNF- 伪) and monocytes in peripheral blood of children with HSP were detected. To investigate the role of TLR9-My D88-NF- 魏 B signaling pathway in the pathogenesis of HSP, and to provide a basis for the search of upstream targets for the treatment of HSP. Methods from June 2015 to December 2016, 40 children with HSP in pediatrics outpatient and hospitalized acute phase in the second affiliated Hospital of Zhengzhou University were selected as the study objects, and 35 children who had recovered HSP in the same period were selected as the normal control group. The plasma IL-6,TNF- 伪 levels were detected by ELISA method, independent t-test and Pearson correlation test were used for the comparison between the two groups. Results the expression of TLR9,My D88 and NF- 魏 B m RNA in peripheral blood monocytes of children with 1.HSP was significantly higher than that of the control group (t = 5.68, P 0.01, P = 5.42, P 0.01, n = 112.95, P 0.01), and there was a significant difference in the expression of TLR9,My D88 and NF- 魏 B m RNA between the two groups. The level of plasma IL-6,TNF- 伪 in 2.HSP group was significantly higher than that in control group (t = 5.11, P 0.01, P = 9.26, P 0.01), and the difference was statistically significant. There was a positive linear correlation between TLR9 and My D88 TLR9 and NF- 魏 By D88 with NF- 魏 B and IL-6 and NF- 魏 B with TNF- 伪 in 3.HSP group. (P 0.01, P 0. 97, P 0. 01). Conclusion the level of NF- 魏 B m RNA,IL-6,TNF- 伪 in TLR9,My D 88 in HSP children is significantly higher than that in control children. The upstream factor TLR9,My D88 NF- 魏 B has a synchronous correlation with the release of IL-6,TNF- 伪. The TLR9-My D88-NF- 魏 B signaling pathway is involved in the pathogenesis of HSP and plays an important role in it.
