住院新生儿维生素D水平调查及影响因素分析

【摘要】：目的调查住院新生儿维生素D营养状况,分析相关影响因素,指导孕母及新生儿维生素D的合理补充。方法以该院2014年1-12月2 199例住院新生儿为研究对象,在家属知情同意的情况下,采集患儿静脉血2mL,采用同位素稀释超高效液相色谱串联质谱法对25(OH)D水平进行检测并对对影响因素进行分析。结果最终共送检标本2 108例,送检率95.9%。参与调查的住院新生儿25(OH)D水平(14.6±6.6)ng/mL,异常2 036例,占96.6%,其中维生素D缺乏的新生儿1 689例,维生素D不足的新生儿347例。在不同胎龄、胎数、小于胎龄儿、适于胎龄儿等方面,维生素D水平差异无统计学意义(P0.05)。冬春季出生患儿维生素D水平明显低于夏秋季出生者,差异有统计学意义(P0.05)。96.6%的患儿维生素D水平明显降低,其中产前母体有异常情况的占61.0%。结论该院住院新生儿维生素D普遍低于正常,冬春季出生的住院新生儿较易表现出维生素D缺乏的情况,新生儿维生素D水平与母体情况密切相关。
[Abstract]:Objective to investigate the nutritional status of vitamin D in hospitalized newborns and analyze the related factors to guide the rational supplement of vitamin D in pregnant mothers and newborns. Methods A total of 2,199 hospitalized newborns from January to December 2014 in the hospital were studied. The venous blood samples were collected with the informed consent of the family members. Isotope dilution ultrahigh performance liquid chromatography tandem mass spectrometry was used to detect 25 (OH) D level and the influencing factors were analyzed. Results A total of 2 108 samples were submitted, and the examination rate was 95. 9%. There were 2 036 (96. 6) cases with abnormal ng/mL, of 25 (OH) D (14.6 卤6. 6) in the hospitalized newborns, including 1 689 neonates with vitamin D deficiency and 347 neonates with vitamin D deficiency. There was no significant difference in vitamin D level between different gestational age, fetal number, smaller gestational age and suitable gestational age (P0.05). Vitamin D levels in children born in winter and spring were significantly lower than those born in summer and autumn, the difference was statistically significant (P0.05). Conclusion Vitamin D of hospitalized newborns is generally lower than that of normal neonates born in winter and spring. Vitamin D level of newborns is closely related to maternal condition.
