肺炎支原体感染的哮喘患儿血清CD40L、VCAM-1水平变化及其意义

【摘要】:目的探讨肺炎支原体(MP)感染的支气管哮喘患儿血清CD40L与血管细胞黏附分子-1(VCAM-1)水平的变化及其意义。方法选择59例支气管哮喘患儿作为观察组,入院后行MP-lg M检测,根据MP是否阳性分为MP感染并支气管哮喘组(MP感染哮喘组)30例、单纯支气管哮喘组(单纯哮喘组)29例。另选择同期正常体检的儿童27例作为对照组。MP感染哮喘组中根据MP-Ig M抗体效价滴度分为低滴度11例、中滴度10例、高滴度9例三个水平。清晨抽取三组空腹静脉血4 m L,离心取上清液,采用酶联免疫法测定血清CD40L及VCAM-1水平。分析MP感染哮喘患儿血清CD40L与VCAM-1水平的相关性。结果单纯哮喘组及MP感染哮喘组血清CD40L、VCAM-1水平均高于对照组(P均0.05),MP感染哮喘组血清CD40L、VCAM-1水平均高于单纯哮喘组(P均0.05)。高滴度及中滴度患儿血清CD40L、VCAM-1水平均高于低滴度患儿(P均0.05)。高滴度及中滴度患儿间血清CD40L与VCAM-1水平差异无统计学意义(P0.05)。MP感染哮喘患儿血清CD40L水平与VCAM-1水平呈正相关(r=0.634,P0.01)。结论哮喘患儿存在MP感染时,血清CD40L及VCAM-1水平可进一步升高,从而加重呼吸道的慢性炎症过程,进而加重支气管哮喘。
[Abstract]:Objective to investigate the changes and significance of serum CD40L and vascular cell adhesion molecule-1 (VCAM-1) levels in children with mycoplasma pneumoniae (MP) infection. Methods 59 children with bronchial asthma were selected as observation group. MP-lg M was detected after admission. According to whether MP was positive or not, 30 cases were divided into MP infection and bronchial asthma group (MP infection asthma group). Simple bronchial asthma group (simple asthma group) 29 cases. According to the titers of MP-Ig M antibody in MP infected asthma group, 11 cases were low titre, 10 cases were middle titre and 9 cases were high titre, which were divided into three levels: low titre (n = 11), middle titre (n = 10) and high titre (n = 9). Fasting venous blood (4 mL) was taken from three groups in the morning and the supernatant was centrifuged. Serum CD40L and VCAM-1 levels were measured by enzyme linked immunosorbent assay (Elisa). To analyze the correlation between serum CD40L and VCAM-1 levels in children with MP infection. Results the levels of serum CD40L,VCAM-1 in asthma group and MP infection group were higher than those in control group (P0.05). The serum CD40L,VCAM-1 level in asthma group was higher than that in asthma group (P0.05). The levels of serum CD40L,VCAM-1 in high and medium titers were higher than those in low titers (P0.05). There was no significant difference in serum CD40L and VCAM-1 levels between high titers and middle titers (P0.05). There was a positive correlation between serum CD40L levels and VCAM-1 levels in MP infected asthmatic children (r = 0.634, P0.01). Conclusion Serum CD40L and VCAM-1 levels can be further increased in asthmatic children with MP infection, which can aggravate the chronic inflammation process of respiratory tract and further aggravate bronchial asthma.
