16~42周胎儿脊髓颈膨大发育的MRI分析

【摘要】:目的利用3.0T MRI观察正常胎儿标本脊髓颈膨大最大横断面及其相对位置关系随孕周变化的规律。方法随机选取44例16~42孕周胎儿标本,行3D-T2WI和T2WI-SPC序列扫描,对所得图像三维重建,观察及测量以下参数:1颈膨大最大横断面面积:选取C4-T1脊神经区间内各椎体中心及椎间盘中心水平分别测量颈膨大横断面面积,选取最大面积,绘制最大横断面面积随孕周变化的生长曲线;2颈膨大最大横断面与椎体的相对位置关系;3比较性别间差异。结果 1颈膨大最大横断面积随孕周呈线性增加,与孕周的相关方程分别为:最大横断面积(cm2)=0.01×孕周-0.12,R2=0.91,P0.05;2颈膨大最大横断面与椎体相对位置关系:16~28孕周胎儿颈膨大最大横断面约位于C5/6椎间盘中心水平,29~42孕周胎儿颈膨大横断面约位于C5~C5/6中心水平,此位置随孕周增加而逐渐轻度上移;3颈膨大最大横断面面积与性别没有明显统计学差异(P0.05)。结论高场磁共振可准确反映胎儿颈膨大最大横断面积及相对位置与孕周变化的规律,建立了正常胎儿脊髓颈膨大生长发育的磁共振影像图谱及数据库。
[Abstract]:Objective to observe the relationship between the maximum transverse section of cervical enlargement of spinal cord and its relative position with gestational weeks in normal fetal specimens with 3.0T MRI. Methods 44 cases of 1642 gestational week fetuses were randomly selected. 3D-T2WI and T2WI-SPC sequences were performed to reconstruct the images. The following parameters were observed and measured: the maximum cross-sectional area of cervical enlargement was measured at the level of vertebral body center and intervertebral disc center in C4-T1 spinal nerve region, and the maximum area was selected. The growth curve of maximum cross-sectional area with gestational age was drawn. (2) the relative position relationship between the maximum transverse section of cervical expansion and the vertebral body; Results 1the maximum cross-sectional area of cervical swelling increased linearly with gestational weeks, and the correlation equations with gestational weeks were: maximum cross-sectional area (cm _ 2) was 0.01 脳 gestational weeks (-0.12). R2O0.91, P0.05; 2the relationship between the maximum transverse section of cervical enlargement and the relative position of the vertebral body is that the largest cross-sectional section of the cervical enlargement of the fetus is located at the central level of C5 / 6 intervertebral disc at the gestational week. The transverse section of cervical enlargement at gestational week 2942 was located at the center of C5 / 5 / 6, and the position moved up slightly with the increase of gestational week. 3There was no significant difference between the maximum cross-sectional area of cervical enlargement and gender. Conclusion High field magnetic resonance imaging can accurately reflect the rule of the maximum cross-sectional area, relative position and gestational week of fetal neck enlargement. The magnetic resonance imaging atlas and database of normal fetal cervical expansion were established.
