中心气道肿瘤患者肺部感染能谱CT的诊断研究

【摘要】:目的探讨中心气道肿瘤患者肺部感染病原菌分布特征及能谱CT的诊断,为感染的防治提供参考。方法回顾性分析2011年7月-2014年7月医院收治的95例中心气道肿瘤合并肺部感染患者临床资料,分析感染患者病原菌特征;所有患者均行能谱CT平扫和动脉期增强扫描,观察患者肺部感染病变及非感染性病变,采用平扫和增强动脉期40keV的CT值、碘含量、水含量及能谱曲线斜率。结果 95例感染患者分离出103株病原菌,其中革兰阳性菌64株占62.1%,革兰阴性菌35株占34.0%,真菌4株占3.9%;平扫时肺部感染性病变与非感染性病变间40keV的CT值、能谱曲线斜率、碘含量、水含量比较差异均无统计学意义;动脉期非感染性病变40keV的CT值、能谱曲线斜率、碘含量显著高于感染性病变,差异有统计学意义(P0.05),感染性与非感染性病变水含量比较,差异无统计学意义。结论中心气道肿瘤肺部感染的主要病原菌为革兰阳性菌;能谱CT能够对感染组织进行定性、定量分析,为疾病的诊断提供了一种新颖的影像学诊断方法。
[Abstract]:Objective to investigate the distribution of pathogenic bacteria of pulmonary infection in patients with central airway tumor and the diagnosis of energy dispersive CT (EDS). Methods the clinical data of 95 patients with central airway tumor complicated with pulmonary infection from July 2011 to July 2014 were analyzed retrospectively. All the patients were performed EDS plain scan and arterial phase enhanced scan. The pulmonary infection and non-infective lesions were observed. The CT value and iodine content of 40 Kev in the arterial phase were measured by plain scan and enhanced arterial phase. Results 103 strains of pathogenic bacteria were isolated from 95 infected patients. There were 64 Gram-positive bacteria (62.1%), 35 Gram-negative bacteria (34.0) and 4 fungi (3.9%). There was no significant difference in CT value, energy spectrum curve slope, iodine content and water content between pulmonary infectious lesions and non-infectious lesions during plain scan. The CT value, slope of energy spectrum curve and iodine content of 40 Kev in arterial non-infectious lesions were significantly higher than those in infective lesions, the difference was statistically significant (P 0.05), and the water content of infectious and non-infectious lesions was higher than that of non-infective lesions. Conclusion Gram-positive bacteria are the main pathogens of pulmonary infection in central airway tumors, and EDS CT can provide a novel imaging diagnosis method for the diagnosis of the disease by qualitative and quantitative analysis.
