三维超声造影对肝癌射频治疗的评价

【摘要】：目的探讨三维超声造影(3D-CEUS)对肝癌射频微创治疗效果的评价。方法对102例肝癌患者131个病灶进行超声引导下射频消融微创治疗,治疗前后分别进行3D-CEUS和二维超声造影(2D-CEUS)检查。结果治疗前3D-CEUS对肿瘤三维立体形态及肿瘤血管空间关系的显示率为92.3%,而2D-CEUS显示率为0.0%(P0.05)。3D-CEUS对肿瘤滋养血管显示率100%,而2D-CEUS显示率为57.3%(P0.05)。治疗后3D-CEUS对消融不全病灶残余血管的显示率为100%,而2D-CEUS显示率为9.3%(P0.05)。结论 3D-CEUS可良好显示肿瘤立体空间形态、肿瘤血管空间灌注过程及肿瘤滋养血管,有助于临床精确制订消融方案,对消融术后疗效评估、随访有较高临床价值。
[Abstract]:Objective to evaluate the curative effect of radiofrequency minimally invasive radiofrequency ablation (RFA) in 102 patients with hepatocellular carcinoma (HCC) by 3D-CEUS.Methods one hundred and thirty-one lesions in 102 patients with hepatocellular carcinoma were treated with radiofrequency ablation under the guidance of ultrasound. Results before and after treatment, 3D-CEUS and 2D-CEUS were performed respectively. Results before treatment, 3D-CEUS showed the three-dimensional morphology of tumor and the spatial relationship of tumor vessels, while 2D-CEUS showed 100% of tumor nourishing vessels and 2D-CEUS showed 100% of tumor nourishing vessels. After treatment, the display rate of residual blood vessels in incomplete lesions by 3D-CEUS was 100, while that of 2D-CEUS was 9.3%. Conclusion 3D-CEUS can show the three-dimensional space of the tumor well. The process of tumor blood vessel space perfusion and tumor nourishing blood vessel are helpful to make the ablation plan accurately and have high clinical value in evaluating the curative effect after ablation and following up.
