斑点追踪成像技术评价正常儿童左心室局部心肌周向功能

【摘要】:目的探讨正常儿童左心室局部心肌收缩期周向峰值应变(SC)及应变率(SrC)与年龄、心率的关系。方法收集164名正常儿童胸骨旁左心室短轴基底段、中间段及心尖水平二维动态图像,采用GE EchoPAC工作站分析,并获得左心室18节段SC及SrC值,将其与年龄和心率进行回归分析。结果儿童18个节段的周向应变及应变率曲线形态基本一致。基底段、中间段和心尖段的前间隔、后间隔、下壁SC较高,侧壁与后壁较低。基底段、中间段和心尖段不同部位SrC差异有统计学意义(P均0.05)。除基底段和中间段的前间隔、前壁、后间隔和心尖段的后间隔以外,其余节段与年龄存在相关性;除前间隔所有节段、前壁中间段及心尖段、后间隔心尖段外,其余节段SC与心率有相关性;而中间段下壁SC与心率和年龄均有相关性(R~2=0.169,P=0.032)。基底段和中间段的后壁及下壁、中间段的侧壁、心尖段的前间隔、前壁、侧壁、后壁、下壁和后间隔SrC与年龄无相关性,其余节段与年龄存在相关性。基底段的后壁、心尖段的前壁、侧壁、后壁SrC与心率无相关性,其余节段SrC均与心率有相关性(P均0.05)。心尖段的前间隔SrC与心率和年龄均有相关性(R~2=0.053,P=0.044)。结论正常儿童局部心肌SC及SrC存在节段间的差异,且与心率和(或)年龄存在相关性。采用斑点追踪成像技术评价儿童左心室局部心肌SC及SrC时需考虑年龄、心率因素。
[Abstract]:Objective to investigate the relationship between age and heart rate in left ventricular regional systolic peak strain (SCC) and strain rate (SRC) in normal children. Methods two dimensional dynamic images of left ventricular short axis basal segment, middle segment and apical level were collected from 164 normal children. The values of SC and SrC in 18 segments of left ventricle were analyzed by GE EchoPAC workstation. The values of SC and SrC were analyzed by regression analysis with age and heart rate. Results the shape of circumferential strain and strain rate curves of 18 segments of children was basically the same as that of basal segment. The anterior septum, posterior septum, inferior wall SC were higher in middle segment and apical segment, but lower in lateral wall and posterior wall. There were significant differences in SrC between basal segment, middle segment and apical segment (P < 0.05), except for anterior septum and anterior wall of basal segment and middle segment. In addition to posterior septum and apical segment, the other segments were correlated with age, except all segments of anterior septum, middle segment of anterior wall, apical segment of anterior wall, apical segment of posterior septum, and other segments of SC were correlated with heart rate. There was no correlation between SrC of basal and middle segments, lateral wall of middle segment, anterior septum of apical segment, anterior wall, lateral wall, posterior wall, inferior wall and posterior septum. There was no correlation between SrC and heart rate in the posterior wall of basal segment, anterior wall of apical segment, lateral wall, posterior wall, and posterior wall. The other segments of SrC were correlated with heart rate (P < 0.05), and the anterior septal SrC of the apical segment was correlated with heart rate and age. Conclusion there is a significant difference between SC and SrC in the local myocardium of normal children. The age and heart rate factors should be taken into account in the evaluation of left ventricular regional myocardial SC and SrC by dot-tracing imaging.
