磁共振弥散加权成像在股骨头坏死诊断中的应用价值

【摘要】:目的研究磁共振弥散加权成像及ADC值在股骨头坏死诊断及分期中的意义。方法收集股骨头坏死患者64例作为实验组,共80髋,男36例,女28例,年龄19-79岁,平均年龄44.3岁。病程2个月-17年不等。随机选取健康志愿者40例,共80髋,男23例,女17例,年龄20-76岁,平均年龄51.4岁,作为对照组。全部研究对象均采用GE 1.5T扫描机进行常规序列及DWI(b值取800 s/mm2)序列扫描,全方向弥散,获得ADC图,测得股骨头感兴趣区域ADC值,并对结果进行统计学分析。结果DWI可以清晰显示股骨头坏死区域及正常股骨头解剖结构。股骨头坏死区域ADC值为(7.01±1.41)×10-4mm2/s,其中只伴骨髓水肿组、只伴髋关节积液组、同时伴骨髓水肿和髋关节积液组、不伴骨髓水肿和髋关节积液组的股骨头ADC值分别为(7.26±1.26)×10-4mm2/s、(7.43±1.05)×10-4mm2/s、(7.63±0.77)×10-4mm2/s、(4.75±0.82)×10-4mm2/s;正常人股骨头ADC值为(2.94±0.76)×10-4mm2/s。结论MR DWI序列可以区分坏死股骨头及正常人股骨头;缺血坏死股骨头ADC值明显高于正常股骨头ADC值;伴骨髓水肿和/或髋关节积液的坏死股骨头ADC值增高;ADC值对于股骨头坏死的早期诊断及分期有一定价值。
[Abstract]:Objective to study the significance of magnetic resonance diffusion weighted imaging and ADC value in the diagnosis and staging of femoral head necrosis. Methods 64 cases of femoral head necrosis were collected as experimental group, including 80 hips, 36 men, 28 women, 19-79 years old and 44.3 years old. The course of disease was 2 months -17 years. 40 cases of healthy volunteers were randomly selected, 80 hips, 23 men, 17 cases, year. The age of 20-76 years, the average age of 51.4 years old, as the control group, all the subjects were used GE 1.5T scanner for routine sequence and DWI (b value 800 s/mm2) sequence scanning, the whole direction was dispersed, the ADC map was obtained, the ADC value in the region of interest of the femoral head was measured, and the results were statistically analyzed. Results DWI can clearly show the area of avascular necrosis of the femoral head and the result of DWI. Normal femoral head anatomy. The ADC value of femoral head necrosis was (7.01 + 1.41) x 10-4mm2/s, with only bone marrow edema group, only with hip joint effusion group, combined with bone marrow edema and hip joint effusion group, without bone marrow edema and hip joint effusion, the ADC value of the femoral head was (7.26 + 1.26) x 10-4mm2/s, (7.43 + 1.05) x 10-4mm2/s, (7.63 +). 0.77) x 10-4mm2/s, (4.75 + 0.82) x 10-4mm2/s; normal human femoral head ADC value of (2.94 + 0.76) x 10-4mm2/s. conclusion MR DWI sequence can distinguish the femoral head and normal human femoral head; ischemic necrosis of femoral head ADC value is significantly higher than the normal femoral head ADC value; with bone marrow edema and / or hip joint accumulation of necrotic femoral head ADC value; ADC value of the value; ADC value It is valuable for early diagnosis and staging of femoral head necrosis.
