乳腺癌患者癌灶动态增强磁共振成像表现与ER、PR、Akt、p27表达的相关性

【摘要】:目的探讨乳腺癌动态增强磁共振成像(DCE-MRI)表现与ER、PR、Akt、p27表达的相关性,探讨用乳腺癌DCE-MRI表现预测乳腺癌ER、PR、Akt和p27表达的可能性。方法回顾150例乳腺癌患者156个病灶的DCE-MRI表现,包括病灶最大径、病灶形状、病灶边缘情况、DCE-MRI强化形式、时间信号强度曲线(TIC)类型、早期增强率。采用免疫组化Elivision法检测156个乳腺癌病灶的ER、PR、Akt、p27表达情况。将病灶按DCE-MRI病灶最大径、病灶形状、病灶边缘情况、DCE-MRI强化形式、TIC类型、早期增强率分为不同亚组,采用χ~2检验比较各亚组ER、PR、Akt、p27阳性表达率。采用Spearman相关性分析法计算病灶最大径、病灶形状、病灶边缘情况、DCEMRI强化形式、TIC类型、早期强化率与ER、PR、Akt和p27表达的相关系数(r)和P值。结果本组乳腺癌病灶最大径≤2 cm者84个、2 cm者72个;病灶形态类圆形者23个、分叶形51个、不规则形62个;病灶边缘光滑9个、不规则62个、毛刺85个;DCE-MRI强化形式为均匀强化者31个、不均匀强化者80个、环形强化者45个;TIC类型Ⅰ型10个、Ⅱ型59个、Ⅲ型87个;早期强化率50%者11个、50%~100%者66个、100%者79个。156个乳腺癌病灶中ER表达阳性122个(78.21%),PR表达阳性122个(78.21%),Akt表达阳性104个(66.67%)、p27阳性106个(67.95%)。肿病灶边缘情况与ER表达有关(χ~2=4.662,P0.05)。病灶边缘情况、DCE-MRI强化形式与PR表达有关(χ~2=4.515、6.017,P均0.05)。早期增强率与Akt表达有关(χ~2=6.211,P0.05)。病灶最大径、病灶形态、TIC类型与p27表达有关(χ~2=5.932、4.851、5.828,P均0.05)。Spearman相关性分析结果显示,病灶边缘情况与ER阳性表达有关(r为0.594,P0.05),病灶边缘情况、DCE-MRI强化形式与PR阳性表达有关(r分别为6.13、-0.558,P均0.05),早期增强率与Akt阳性表达有关(r为0.587,P0.05),乳腺癌病灶最大径、病灶形态、TIC类型与p27阳性表达有关(r分别为0.721、0.689、0.707,P均0.05)。结论乳腺癌病灶边缘情况与ER阳性表达有关,病灶边缘情况、DCE-MRI强化形式与PR阳性表达有关,早期增强与Akt阳性表达有关,乳腺癌病灶最大径、病灶形态、TIC类型与p27阳性表达有关。
[Abstract]:Objective to investigate the correlation between dynamic enhanced magnetic resonance imaging (DCE-MRI) of breast cancer and the expression of Aktp27 in ERT PRA, and to explore the possibility of predicting the expression of Akt and p27 by DCE-MRI in breast cancer. Methods the DCE-MRI findings of 156 lesions in 150 patients with breast cancer were retrospectively reviewed, including the maximum diameter of the lesion, the shape of the lesion, the enhanced form of DCE-MRI, the time signal intensity curve and the early enhancement rate. Immunohistochemical Elivision method was used to detect the expression of ERP P 27 in 156 breast cancer lesions. According to the maximum diameter, shape and edge of DCE-MRI, the early enhancement rate was divided into different subgroups, and the positive expression rate of ERP PRA AktP27 in each subgroup was compared by 蠂 ~ 2 test. Spearman correlation analysis was used to calculate the maximum diameter, shape and edge of the lesion. The correlation coefficient of early enhancement rate with the expression of Akt and p27 in ERV PRT and p27 was calculated. Results in this group, the largest diameter of breast cancer was 84 or 2 cm in diameter, 23 lesions were round in shape, 51 in lobular shape, 62 in irregular shape, 9 in smooth margin and 62 in irregular shape.
