骶骨肿瘤动脉术前选择性栓塞的临床意义

【摘要】：目的探讨选择性术前栓塞骶骨肿瘤动脉的临床应用价值。方法对71例骶骨肿瘤切除术前患者行1~6次[平均(1.45±1.01)次]肿瘤动脉栓塞。记录患者术中和术后24h出血量和与栓塞相关的并发症。单次栓塞与多次栓塞后手术切除患者的出血量比较采用独立样本t检验。结果 54例患者在1次动脉栓塞后行肿瘤切除术;17例患者于2~6次栓塞后行肿瘤切除术。栓塞后手术患者术中和术后24h平均出血量(2457.75±1508.14)ml。单次栓塞后接受手术治疗患者的平均出血量(2200.00±1350.47)ml,多次栓塞后手术患者的平均出血量为(3276.47±1724.29)ml,差异无统计学意义(t=1.588,P=0.212)。骨巨细胞瘤、脊索瘤、软骨肉瘤和转移瘤患者出血量差异无统计学意义(F=0.592,P=0.623)。结论骶骨肿瘤切除术前的动脉栓塞可有效减少术中和术后出血,保证手术顺利进行。
[Abstract]:Objective to evaluate the clinical value of selective preoperative embolization of sacral tumor arteries. Methods Seventy-one patients with sacral neoplasms were treated with arterial embolization for 6 times (mean 1.45 卤1.01). The volume of bleeding and the complications associated with embolism were recorded during and 24 hours after operation. Single embolization and multiple embolization were used to compare the amount of blood loss in patients undergoing surgical excision with independent sample t test. Results tumor excision was performed in 54 patients after one arterial embolization and tumor excision was performed in 17 patients after 2 times of embolization. The mean blood loss was 2457.75 卤1508.14 ml during operation and 24 hours after embolization. The average volume of blood loss was 2200.00 卤1350.47ml after single embolization, and 3276.47 卤1724.29ml after multiple embolization. There was no significant difference between the two groups. There was no significant difference in blood loss between giant cell tumor of bone, chordoma, chondrosarcoma and metastatic tumor. Conclusion Arterial embolization before sacral tumor resection can effectively reduce intraoperative and postoperative bleeding and ensure smooth operation.
