高分辨率MR相位差增强成像技术对健康成人Gennari纹的初步研究

【摘要】:研究目的 应用高分辨率相位差增强成像(phase difference enhanced imaging, PADRE)技术研究正常成人Gennari纹的显示情况,并测算初级视觉皮质及视辐射的相位值,探讨利用PADRE技术研究活体大脑皮质纤维构筑的可行性。 资料与方法 本研究经山东省医学影像学研究所伦理学委员会同意并获得受检者知情同意。研究对象为34名健康志愿者,其中男20例,女14例,年龄21-50岁,平均年龄30.4±8.0岁。采用荷兰Philips Achieva TX3.0T超导MR扫描仪,8通道相控阵头线圈为接收线圈。对所有受检者行高分辨率PADRE技术颅脑成像,并对获得的原始图像分别进行组织增强(tissue enhancement, TE)、血管增强(vessel enhancement, VE)及相位增强(PADRE)三种模式的重建。 (1)正常成人Gennari纹的显示情况评价在未知图像重建模式情况下,由2位具有高年资神经放射诊断经验的医师分别评估所有受试者TE、VE及PADRE图像上枕叶灰白质的显示情况,重点对3组图像上Gennari纹的显示情况进行独立评价,评分标准为:0分:未见显示Gennari纹;1分:不能明确辨认Gennari纹,但隐约可见;2分:可以辨认Gennari纹,但走行欠连续或不能显示其在皮质中走行位置;3分,可以明确辨认Gennari纹,且走行连续,可分辨在皮质中的位置;此外,经初级视觉皮质垂直于皮质表面划一直线感兴趣区(region of interest, ROI)并计算该ROI的信号强度曲线,观察在该曲线上Gennari纹走行处有无信号强度的降低。对于2名阅片者间评分结果行Kappa一致性检验。对于PADRE、TE及VE图像上Gennari纹的评分值,采用Wilcoxon符号秩和检验进行分析;p0.05为差异具有统计学意义。 (2)初级视觉皮质及视辐射的相位值测量将PADRE原始数据以DICOM格式转存至专用电脑上后,采用SPIN软件进行高通滤波,得到“滤过”的相位图(filtered phase image, FPI),在FPI上选取两侧距状沟前、后的皮质以及视辐射走行处设置圆形ROI测算初级视觉皮质及视辐射的相位值。对于所得相位值先行正态性检验,对服从正态分布的相位值进行配对设计数值变量资料的t检验。p0.05为差异具有统计学意义。 结果 36名志愿者经PADRE技术重建的3组图像中皮质表现为相对高信号,白质则为较低信号;3组图像均可不同程度地显示Gennari纹,表现为与初级视觉皮质表面平行的低信号带。TE图像中灰白质对比显示最好,可识别Gennari纹在皮质中的走行位置,但是对Gennari纹的显示较淡;VE图像中灰白质对比较差,不能识别Gennari纹在皮质中的走行位置,但是对Gennari纹的显示较清晰;PADRE图像中灰白质对比较好,可清晰显示Gennari纹,亦可分辨Gennari纹在皮质中的走行位置。在三组图像中ROI的信号强度曲线上,可以观察到在Gennari纹的走行处存在明显的信号强度的降低。 2名评阅者对TE.VE及PADRE图像上Gennari纹的评分值分别为1.79±0.44、1.84±0.37、2.84±0.37,1.76±0.43、1.82±0.38、2.7±0.41,评阅者间的评分结果一致性较好(κ=0.823)。2名评阅者在TE、VE及PADRE图像上对Gennari纹的平均评分值为1.78±0.43、1.83±0.38、2.8±0.39,PADRE图像上Gennari纹的评分值与其他2组图像的评分值比较差异具有统计学意义,PADRE图像对Gennari纹的显示优于TE和VE图像(Z=-13.337,p0.01;Z=-13.458,p0.01)。 左、右两侧初级视觉皮质及视辐射的相位值分别为0.2184±0.080,0.216±0.080;-0.45±0.113,-0.436±0.128,双侧间比较差异无统计学意义(t=0.150,p0.05;t=-1.136,p0.05);双侧初级视觉皮质和视辐射的平均相位值为0.21±0.068、-0.447±0.106,二者间差异具有统计学意义(t=29.681,p0.01)。 结论 PADRE技术可以显示Gennari纹,尤以PADRE图像显示最佳,提示PADRE技术可用于活体大脑皮质内有髓神经纤维的MR成像,这为研究活体大脑皮质区纤维构筑与其功能的相关性提供了一种新方法。
[Abstract]:research objective
The phase difference enhanced imaging (PADRE) technique was used to study the display of Gennari lines in normal adults, and to calculate the phase values of the primary visual cortex and the visual radiation, and to explore the feasibility of using PADRE to study the construction of the living cerebral cortex fibers.
Information and methods
The study was approved by the ethics committee of the Institute of medical imaging of Shandong province. The subjects were 34 healthy volunteers, including 20 men, 14 women, 21-50 years old, and the average age was 30.4 + 8 years. The Holland Philips Achieva TX3.0T superconducting MR scanning instrument was used and the 8 channel phased array head coil was the receiving coil. The subjects were treated with high resolution PADRE technique for brain imaging, and the original images were reconstructed by tissue enhancement (tissue enhancement, TE), vascular enhancement (vessel enhancement, VE) and phase enhancement (PADRE), respectively.
(1) the display situation of Gennari pattern in normal adults was evaluated under the condition of unknown image reconstruction. 2 patients with high annual neuroradiation diagnosis experience were evaluated respectively for the display of the occipital gray matter in the TE, VE and PADRE images of all the subjects, and the display situation of the Gennari pattern on the 3 group images was evaluated independently. : 0 points: no display of Gennari lines; 1 points: can not clearly identify Gennari lines, but vaguely visible; 2: can identify Gennari lines, but the line is not continuous or can not show its position in the cortex; 3 points can clearly identify the lines, and walk continuously, can distinguish the position in the cortex; moreover, through the primary visual cortex vertical On the cortical surface, the region of interest (ROI) is calculated and the signal intensity curve of the ROI is calculated, and the decrease of the signal intensity is observed on the line of Gennari lines on this curve. The Kappa consistency test is performed for the score results between the 2 film readers. For PADRE, TE and VE images, the Gennari marks are used for the Wilcoxon symbols. Rank sum test was used for analysis; P0.05 was statistically significant.
(2) the primary visual cortex and the phase value measurement of the visual radiation are transferred to the special computer in the DICOM format. The SPIN software is used for high pass filtering, and the "filtered" phase diagram (filtered phase image, FPI) is obtained, and the circular ROI measurement is set on the FPI on the FPI, and the posterior cortex and the optic radiation walk. The phase values of the primary visual cortex and the visual radiation. For the first normal test of the obtained phase values, the t test of the data of the paired design numerical variables for the phase values of the normal distribution is statistically significant.
Result
The 3 groups of 36 volunteers, reconstructed by PADRE technique, showed relatively high signal in the cortex, and the white matter was a lower signal. The 3 group images could display the Gennari pattern in varying degrees, showing the best contrast of gray matter in the low signal band.TE images parallel to the primary visual cortex surface, and could identify the walking position of the Gennari lines in the cortex. But the display of Gennari lines is weak; the gray matter in the VE image is poor and can not identify the walking position of the Gennari lines in the cortex, but the display of the Gennari pattern is clearer; the gray matter in the PADRE image is better, can clearly display the Gennari lines, and can also distinguish the walking position of the Gennari lines in the cortex. In the three groups of images, ROI On the signal intensity curve, it is observed that there is a significant decrease in signal intensity at the Gennari stripe.
The scores of the 2 reviewers on the TE.VE and PADRE images were 1.79 + 0.44,1.84 + 0.37,2.84 + 0.37,1.76 + 0.43,1.82 + 0.38,2.7 + 0.41 respectively. The scores of the judges were in good agreement (KB =0.823), and the average score of the.2 names was 1.78 + + 0.39. The difference between the score value of the Gennari pattern on the image and the score of the other 2 sets of images has statistical significance. The display of the PADRE image to the Gennari pattern is superior to the TE and VE images (Z=-13.337, P0.01; Z=-13.458, P0.01).
The phase values of primary visual cortex and optic radiation on both left and right sides were 0.2184 + 0.080,0.216 + 0.080, -0.45 + 0.113 and -0.436 + 0.128. There was no significant difference between bilateral and bilateral (t=0.150, P0.05; t=-1.136, P0.05), and the average phase values of bilateral primary visual cortex and visual radiation were 0.21 + 0.068, -0.447 + 0.106, and there were differences between two. Statistical significance (t=29.681, P0.01).
conclusion
PADRE technique can display Gennari lines, especially in PADRE images, suggesting that PADRE technology can be used in MR imaging of myelinated nerve fibers in the cerebral cortex of the living body, which provides a new method for the study of the correlation between the fibrous architecture and its function in the cerebral cortex of the living body.
