磁共振弥散张量成像在帕金森病早期诊断以及定量分析中的临床应用价值

【摘要】:目的探讨采取磁共振弥散张量成像技术对帕金森病(PD)患者实施早期诊断及定量分析的临床意义。方法将2013-2014年在广东省第二人民医院就诊并确诊为PD患者30例以及健康对照组30例进行颅脑10个方向磁共振弥散张量成像(DTI)扫描,并分析其颅脑两侧黑质、丘脑、脑桥、小脑上脚及中脚的各向异性分数(FA)值及表观散系数(ADC)值变化情况。结果经统计学分析,在该研究的感兴趣区域内,两组间除黑质区FA值有差异外(t黑质=4.42,P0.05),其他区域差异无统计学意义,且黑质区不同部位差异有所不同,其中以黑质头部FA值差异最大(t头部=4.52,P0.05),且ROC曲线面积为0.836;同时,PD组小脑上脚ADC值与健康组也具有一定差异(t小脑上脚=2.37,P=0.028)。结论 DTI技术在帕金森病早期诊断中具有实际应用价值,尤其是当黑质头部的FA值变化幅度较大时,帕金森病发病率更高。
[Abstract]:Objective to investigate the clinical significance of early diagnosis and quantitative analysis of (PD) patients with Parkinson's disease by magnetic resonance diffusion Zhang Liang imaging. Methods from 2013 to 2014, 30 patients with PD diagnosed at the second people's Hospital of Guangdong Province and 30 healthy controls were examined by 10 directions diffusion-weighted Zhang Liang (DTI) scan, and their bilateral substantia nigra, thalamus and pons were analyzed. The changes of anisotropic fraction (FA) value and apparent dispersion coefficient (ADC) of the upper and middle feet of the cerebellum. Results by statistical analysis, in the region of interest in the study, there was a difference in FA value between the two groups except in the substantia nigra (t substantia nigra 4.42), but there was no significant difference in other regions, and the difference in different parts of the substantia nigra was different between the two groups. The FA value of the substantia nigra head was the largest (t head 4.52% P0.05), and the area of ROC curve was 0.836, and the ADC value of the superior cerebellum of PD group was also different from that of the healthy group (2.37% P0. 028). Conclusion DTI technique has practical value in the early diagnosis of Parkinson's disease, especially when the change of FA value of substantia nigra head is large, the incidence of Parkinson's disease is higher.
