超声弹性应变率比值在TI-RADS 4~6类甲状腺结节鉴别诊断中的应用

【摘要】:目的 :探讨超声弹性应变率比值在TI-RADS 4~6类甲状腺结节中的应用价值。方法:常规超声观察303例(共324个结节)甲状腺结节的二维声像图特征,依据甲状腺影像数据报告与评分系统(TI-RADS)对甲状腺结节进行分类,并进行超声弹性成像,绘制ROC曲线确定弹性应变率比值最佳鉴别诊断界值。将137个TI-RADS 4~6类甲状腺结节分组:Ⅰ组TI-RADS 4A类,Ⅱ组TI-RADS 4B类,Ⅲ组TI-RADS 5~6类(6类中除去已经病理证实者),分析弹性应变率比值在3组病例中的应用价值。结果:弹性应变率比值最佳界值为0.55,弹性应变率比值在Ⅰ组中差异无统计学意义,在Ⅱ、Ⅲ组中差异具有统计学意义。弹性应变率比值鉴别诊断Ⅱ组和Ⅲ组的敏感性、特异性、准确性、阳性预测值分别为90.9%、75.0%、87.5%、93.0%(Ⅱ组)和85.7%、80.0%、84.6%、94.7%(Ⅲ组)。结论 :对于Ⅱ、Ⅲ组,联合应用TI-RADS分类系统与弹性应变率比值具有较高的应用价值,提示恶性结节应少用细针穿刺活检等其他检查,早期进行手术治疗;对于Ⅰ组,弹性应变率应用价值有限,应尽早结合其他检查手段以免延误诊断。
[Abstract]:Objective: to evaluate the value of ultrasonic elastic strain rate ratio (ESR) in 6 types of thyroid nodule (TI-RADS 4). Methods: three hundred and thirty three cases (324 nodule) of thyroid nodule were observed by conventional ultrasound.
