心房纤颤患者二尖瓣反流与左心房血栓形成的危险因素分析

【摘要】:目的旨在探讨非瓣膜性房颤血栓形成的心脏本身因素,二尖瓣反流与左心房血栓形成的关系。方法 212例心房纤颤患者接受经胸超声心动图和经食道超声检查。按左心房有无血栓分为血栓组(50例)和无血栓组(162例)。分析入选病例的超声心动图资料进行分析,对单因素分析中有统计学意义的指标行多因素回归分析。结果发现血栓组和非血栓组左心房内径(LAD)[(48.1±4.7)mmvs.(40.4±3.4)mm]、左室舒张末径(LVEDD)[(50.9±3.7)mmvs.(46.4±3.9)mm]、左室射血分数(LVEF)[(56.5±3.9)%vs.(65.9±5.4)%],组间差异有统计学意义(P0.01),血栓组仅有少量反流,中度以上反流患者无血栓形成。结论 LAD、心功能下降是并发左心房血栓的危险因素。二尖瓣反流是左心房血栓形成的保护性因素。
[Abstract]:Objective to investigate the relationship between mitral regurgitation and left atrial thrombosis in patients with non-valvular atrial fibrillation. Methods 212 patients with atrial fibrillation were examined by transthoracic echocardiography and transesophageal echocardiography. According to left atrial thrombus, 50 cases were divided into thrombus group (50 cases) and no thrombus group (162 cases). The echocardiographic data of the selected patients were analyzed and multivariate regression analysis was performed on the statistically significant indexes in univariate analysis. The results showed that left atrial diameter (LAD) [(48.1 卤4.7) mmvs. (40.4 卤3.4) mm], left ventricular end-diastolic diameter (LVEDD) [(50.9 卤3.7) mmvs. (46.4 卤3.9) mm] and left ventricular ejection fraction (LVEF) [(56.5 卤3.9) vs. (65.9 卤5.4)%] were significantly different between the two groups. Conclusion the decrease of LAD, cardiac function is the risk factor of left atrial thrombosis. Mitral regurgitation is a protective factor for left atrial thrombosis.
