磁共振静脉成像对脑静脉窦血栓诊断价值的Meta分析

【摘要】：目的脑静脉窦血栓形成(CVST)临床表现无特异性,常易漏诊、误诊而延误最佳治疗时机。本文应用Meta分析评价磁共振静脉血管成像(MRV)在CVST中的诊断价值。资料与方法计算机检索Cochrane Library、Pub Med、Embase、Web of Science等外文数据库及中国生物医学文献数据库、维普中文科技期刊数据库、万方、中国知网等中文数据库关于MRV诊断CVST的文献。根据诊断试验准确性研究质量条目评价纳入文献质量,利用Metadisc 1.4、Rev Man 5.0软件对纳入的研究结果进行分析,合并计算敏感度、特异度、诊断比值比,绘制汇总受试者工作特征曲线。结果最终8篇文献12个研究符合纳入标准。Meta分析结果显示,MRV诊断CVST的合并敏感度为0.86(0.80~0.91)、合并特异度为0.87(0.83~0.93)、诊断比值比为29.65(12.48~70.47),汇总受试者工作特征曲线下面积为0.9125。结论 MRV对CVST具有良好的诊断价值,可以作为诊断CVST有效可行的方法。
[Abstract]:Objective the clinical manifestation of cerebral venous sinus thrombosis (CVST) is nonspecific, often missed, misdiagnosed and delayed the best treatment time. Meta analysis was used to evaluate the diagnostic value of magnetic resonance venography (MRV) in CVST. Materials and methods Chinese databases such as Cochrane Library,Pub Med,Embase,Web of Science and Chinese biomedical literature database, Weipu Chinese scientific and technological journal database, Wanfang database and Chinese Zhiwang database on MRV diagnosis of CVST were searched by computer. According to the accuracy of diagnostic test, the evaluation of quality items was included in the literature quality. The results of the study were analyzed by Metadisc 1.4% Rev Man 5.0 software, and the sensitivity, specificity and diagnostic ratio were calculated. Draw a summary of the subject's operating characteristics curve. Results 12 studies in 8 literatures met the inclusion criteria. Meta analysis showed that the combined sensitivity and specificity of MRV in the diagnosis of CVST were 0.86 (0.80 / 0.91) and 0.87 (0.83 / 0.93), respectively. The diagnostic ratio was 29.65 (12.48 / 70.47), and the area under the operating characteristic curve was 0.9125. Conclusion MRV has good diagnostic value for CVST and can be used as an effective and feasible method for diagnosis of CVST.
