增强型血流显像及频谱多普勒对高尿酸血症患者眼动脉的血流动力学研究

【摘要】:目的:采用增强型血流显像(eFlow)及频谱多普勒研究高尿酸血症患者眼动脉(ophthalmic artery,OA)和视网膜中央动脉(central retinalartery CRA)的内径及血流动力学改变,并探讨血流动力学指标和尿酸值的关系,旨在了解高尿酸血症和外周血管疾病的关系。 方法:选择确诊为高尿酸血症的患者101名作为实验组,20名原发性高血压患者和30名健康成年人作为对照组。eFlow观察眼动脉、视网膜中央动脉在眶内的走形情况及内径,频谱多普勒检测眼动脉及视网膜中央动脉的收缩期峰值血流速度(peak of systolic velocity PSV)、舒张末期血流速度(end of diastolic velocity EDV)、血流阻力指数(resistanceindex RI)和搏动指数(pulsatility index PI)。根据确诊高尿酸血症的年限将实验组分为≤5年组和>5年组,采用独立样本t检验比较两组样本均数;再根据有无高血压分为高尿酸血症伴高血压组和不伴高血压组,,所得数据和健康对照组做单因素方差分析,各组间比较采用SNK法,再采用独立样本t检验比较高尿酸血症伴高血压组和原发性高血压组的样本均数;用person,s相关分别分析高尿酸血症伴高血压组和高尿酸血症不伴高血压组的血流动力学指数和尿酸值的相关性。 结果:(1)确诊高尿酸血症>5年组眼动脉的RI(0.68±0.09)、PI(1.3±0.4)较≤5年组(RI:0.63±0.09, PI:1.1±0.3)明显升高(t=3.504,P=0.001;t=3.164,P=0.002),眼动脉的舒张末期血流速度(EDV)>5年组(6.4±3.4) cm/s,较≤5年组(7.5±4.9cm/s)明显降低(t=1.988,P=0.049);(2)高尿酸血症伴高血压组CRA的PSV(11.5±3.5)cm/s,、 EDV(3.7±1.1)cm/s较高尿酸血症不伴高血压组[(PSV:(13.5±4.0)cm/s,EDV:(4.1±1.2)cm/s.)明显降低,RI明显升高(伴高血压组:0.88±1.40,不伴高血压组:0.67±0.08,P<0.05);(3)高尿酸血症伴高血压组眼动脉的RI、PI(0.73±0.68、1.62±0.37)均较原发性高血压组(0.67±0.73、1.48±0.27)升高(t=2.05,P=0.043;t=2.08,P=0.041);(4)RI实验组眼动脉及视网膜中央动脉内径和对照组无明显统计学差异(P>0.05)。(5)高尿酸血症不伴高血压组眼动脉的RI (r=0.274, P=0.007),PI(r=0.318, P=0.001)和血清尿酸值具有相关性,CRA的各项指标和血清尿酸值无明显相关,而高尿酸血症伴高血压组的眼动脉及视网膜中央动脉的各项指标和血尿酸均无明显相关性。 结论:eFlow结合多普勒技术对研究高尿酸血症所致眼动脉血流动力学改变具有较高的临床价值,并提示高尿酸血症可加速眼动脉及视网膜中央动脉的粥样硬化改变,而高尿酸血症合并高血压可进一步加速终末小血管的粥样硬化改变。eFlow结合多普勒技术对预测外周终末小动脉的硬化性改变具有一定的临床价值。
[Abstract]:Objective: to study the internal diameter and hemodynamic changes of ophthalmic artery (ophthalmic artery,OA) and central retinal artery (central retinalartery CRA) in patients with hyperuricemia by enhanced blood flow imaging (eFlow) and spectral Doppler. In order to understand the relationship between hyperuricemia and peripheral vascular diseases, the relationship between hemodynamic parameters and uric acid value was discussed.
