钆塞酸二钠增强磁共振对肝癌TACE术后局部肝功能损伤的评价

【摘要】:目的:探讨应用钆塞酸二钠(Gadolinium ethoxybenzyl diethylenetriamine pentaacetic acid, Gd-EOB-DTPA)增强核磁共振(Magnetic resonance imaging, MRI)扫描评价肝细胞癌(Hepatocellular carcinoma, HCC)行经导管动脉灌注化疗栓塞(Transcatheter arterial chemoembolization, TACE)术后局部肝功能损伤的价值。 方法:对29例HCC的患者于TACE术后3-5天以及随访1-2月后行MR扫描,测量平扫和Gd-EOB-DTPA增强后20分钟(肝细胞期)肿瘤周边(3厘米以内)组织以及正常肝组织的信噪比值(Signal to noise ratio, SNR),将二者进行对比。同时将可能的影响因素,如患者年龄、性别、肿瘤直径、肿瘤血供情况、Chind-Pugh分级和碘油栓塞量与结果进行多因素相关分析。 结果:TACE术后3-5天,平扫肿瘤周边组织的SNR值与正常肝组织的SNR差异无统计学意义(t=1.341, P=0.191), Gd-EOB-DTPA增强后20分钟肿瘤周边组织的SNR值明显低于正常肝组织的SNR值(t=3.732,P0.001)。TACE术后1-2月复查MRI,平扫肿瘤周边组织的SNR值与正常肝组织的SNR值差异无统计学意义(t=0.189,P=0.852); Gd-EOB-DTPA增强后20分钟肿瘤周边的SNR与正常肝组织的SNR值差异同样无统计学意义(t=1.535,P=0.136)。局部肝功能损伤的程度与患者年龄、性别、肿瘤直径、肿瘤血供情况、Chind-Pugh分级均无明显相关性,碘油栓塞量是唯一影响局部肝功能损伤程度的因素。 结论:Gd-EOB-DTPA增强MRI扫描是一种有效评价肝癌TACE术后局部肝功能损伤的方法,对指导进一步治疗有着重要价值。图6幅,表6个,参考文献45篇
