血清中γ-GT及GSH在子痫前期患者中的表达及意义

【摘要】:目的:观察子痫前期轻度及重度患者血清中γ-谷氨酰转肽酶(gamma-glutamyltransferase, γ-GT)及谷胱甘肽(γ-谷氨酰-L半胱氨酸-L-甘氨酸,glutathione, GSH)的表达,以明确其与子痫前期的相关性及意义。 方法:随机选取63例妊娠期高血压疾病患者,根据人民卫生出版社出版的妇产科学第七版教材中妊高症的诊断,将其分为子痫前期重度组31例,子痫前期轻度组32例,以28例同孕周正常妊娠者为对照组,空腹抽外周血,用ELISA法检测血清中GSH水平,用全自动生化分析仪检测血清中γ-GT、血脂、肝功、肾功水平及24-小时尿蛋白,同时观察血压、年龄、孕周等相关指标,比较这些指标在子痫前期重度(sPE组)、轻度(mPE组)和正常妊娠组(Normal)表达的差异。 结果:1.三组年龄、孕周均无显著差异(P0.05)。2.各研究组之间γ-GT、GSH的比较结果:①子痫前期重度组及轻度组血清中γ-GT含量均明显高于正常妊娠组,GSH含量均明显低于正常妊娠组(P0.05)。②子痫前期重度组γ-GT含量高于轻度组,GSH含量低于轻度组(P0.05)。3.研究组之间血脂的比较结果:①子痫前期重度组与正常组相比,重度组中总胆固醇(CHO)、甘油三酯(TG)、低密度脂蛋白(LDL)的含量明显升高,高密度脂蛋白(HDL)明显降低(P0.05)。②子痫前期轻度组与正常组相比,轻度组低密度脂蛋白(LDL)高于正常组,高密度脂蛋白(HDL)低于正常组(P0.05),总胆固醇(CHO)、甘油三酯(TG)与正常组无明显差异(P0.05)。③子痫前期重度组与轻度组相比,重度组中的总胆固醇(CHO)、甘油三酯(TG)、低密度脂蛋白(LDL)含量均高于轻度组,高密度脂蛋白(HDL)低于轻度组(P0.05)。4.根据Pearson相关分析发现,1,-GT与收缩压、舒张压、谷丙转氨酶(ALT)、谷草转氨酶(AST)、总胆固醇(CHO)、低密度脂蛋白(LDL)、血尿酸(UA)、24-小时尿蛋白(24-hour proteinuria)表现为正相关关系(P0.01),但与高密度脂蛋白(HDL)、GSH呈负相关关系(P0.01)。 结论:子痫前期患者中存在不同程度的异常血脂代谢。γ-GT在子痫前期患者中的表达随着疾病的严重程度升高,GSH随着疾病的严重程度降低,二者均与子痫前期具有相关性,可作为评估子痫前期严重程度的指标,运用于临床判断疾病的严重程度。
[Abstract]:Aim: to investigate the expression of gamma-glutamyltransferase (纬 -GT) and 纬 -glutamyltransferase (纬 -GT) and glutathione (GSH) in the serum of mild and severe preeclampsia patients, and to determine the correlation and significance between 纬 -glutamyl transpeptidase (纬 -GT) and preeclampsia. Methods: Sixty-three patients with hypertensive disorder complicating pregnancy were randomly selected. According to the diagnosis of PIH in the teaching material of 7th edition of obstetrics and gynecology published by the people's Health Publishing House, they were divided into three groups: severe preeclampsia group (31 cases) and mild preeclampsia group (32 cases). 28 normal pregnant women of the same gestational age were taken as control group. The serum GSH level was detected by ELISA method, serum 纬 -GT, lipids, liver function, renal function and 24-hour urine protein were detected by automatic biochemical analyzer, and blood pressure was observed at the same time. Age, gestational age and other related indexes were compared between severe preeclampsia group (sPE group), mild group (MPE group) and normal pregnancy group (normal group). Result: 1.Three groups of ages, There was no significant difference in gestational age between the two groups. The results showed that the serum 纬 -GT levels in the severe preeclampsia group and mild eclampsia group were significantly higher than those in the normal pregnancy group and that in the normal pregnancy group were significantly lower than those in the normal pregnancy group and the normal pregnancy group. 2. The results showed that the level of 纬 -GT in the serum of the severe preeclampsia group was significantly lower than that in the normal pregnancy group. The content of 纬 -GT was higher in mild group than that in mild group (P 0.05) .3.The comparison of blood lipids between study group and severe preeclampsia group was compared with that in normal group. The contents of total cholesterol, triglyceride, low density lipoprotein (LDL) and high density lipoprotein (HDL) decreased significantly in the severe group compared with the normal group, and the low density lipoprotein (LDL) level in the mild group was significantly higher than that in the normal group, and that in the mild group of mild preeclampsia was significantly lower than that in the normal group. High density lipoprotein (HDL) was lower than normal group (P 0.05), total cholesterol and triglyceride (TG) were not significantly different from normal group (P 0.05.3) compared with mild group and severe preeclampsia group (P 0.05.3), there was no significant difference between severe preeclampsia group (P 0.05) and mild group (P < 0.05). The contents of total cholesterol, triglyceride, low density lipoprotein (LDLs) in the severe group were higher than those in the mild group, and HDLs were lower than those in the mild group (P0.05. 4). According to the correlation analysis of Pearson, it was found that the contents of TTG, TG, LDLs were higher than those in the mild group, and the results of Pearson correlation analysis showed that they were associated with systolic blood pressure (SBP) and diastolic blood pressure (DBP). Alanine aminotransferase (alt), alanine aminotransferase (alt), glutamic oxalacetic transaminase (AST), total cholesterol (TC), low density lipoprotein (LDLN), 24-hour urinary protein (24-hour) were positively correlated, but negatively correlated with high density lipoprotein (HDLGSH) (P 0. 01). Conclusion: there are abnormal lipid metabolism in preeclampsia patients, and the expression of 纬 -GT in preeclampsia patients decreases with the severity of the disease, both of which are correlated with preeclampsia. It can be used as an index to evaluate the severity of preeclampsia.
