Cx43在羊水过少产妇胎膜及胎盘组织中的表达及其意义

【摘要】:目的:通过探讨Cx43在特发性羊水过少、羊水量正常产妇胎膜和胎盘组织中的定性及相对定量表达情况,分析其在羊水过少发生中的重要地位及其可能的调节机制和作用。方法:选取2014年4月至2015年12月在兰州大学第一医院妇产科经剖宫产生产的足月产妇57例为研究对象,羊水过少产妇27例定为实验组、羊水正常产妇30例为对照组。采取免疫组化SABC方法和RT-PCR技术分别检测Cx43蛋白与m RNA在两组产妇胎膜及胎盘中的表达和定位。结果:1.采取免疫组化SABC方法发现Cx43蛋白在两组产妇胎膜、胎盘中均有表达。2.羊水过少组中Cx43蛋白在胎膜上皮细胞中表达阳性率较对照组明显下调,差异显著(其中?2=5.747,P=0.017),Cx43蛋白在胎盘滋养细胞中表达阳性率亦较对照组明显下调,差异显著(其中?2=3.986,P=0.046)。3.利用RT-PCR法发现Cx43m RNA在两组产妇胎膜、胎盘中均有表达。4.羊水过少组Cx43m RNA在胎膜中的表达是对照组的0.51倍,较之,差异显著(其中t=15.792,P0.01),在胎盘中的表达为对照组的0.54倍,较之,差异显著(其中t=14.782,P0.01)。结论:1.此次研究得出Cx43蛋白及及其m RNA在人类胎膜、胎盘中皆有表达,且主要表达在羊膜上皮细胞、绒毛膜滋养细胞及胎盘血管内皮细胞、胎盘合体滋养细胞。2.羊水过少患者的胎膜和胎盘组织中Cx43蛋白及其m RNA表达较正常组均呈下降趋势。3.Cx43在羊水量过少及羊水量正常产妇胎盘、胎膜中的表达变化可能参与羊水量异常的调节,提示Cx43可能在羊水膜内吸收途径及母胎液体交换过程发挥着重要作用。其在胎膜中的表达机制与在胎盘中的表达机制不同,除对水通透功能外可能与细胞凋亡功能有关,具体功能有待进一步研究。4.羊水过少的新生儿Apgar评分与正常足月新生儿的相比有所降低,经比较有显著差异。据此推测羊水过少可能会影响妊娠结局,增加围产儿缺氧及窒息概率。
[Abstract]:Objective: to investigate the qualitative and relative quantitative expression of Cx43 in fetal membrane and placenta of idiopathic oligohydramnios and normal amniotic fluid. To analyze its important role in the occurrence of oligohydramnios and its possible regulatory mechanism and role. Methods: 57 pregnant women who were born by cesarean section in Department of Gynecology and Obstetrics, first Hospital of Lanzhou University, from April 2014 to December 2015, were selected as subjects. 27 cases of oligohydramnios were selected as experimental group, The expression and localization of Cx43 protein and m RNA in fetal membranes and placentas were detected by immunohistochemical SABC method and RT-PCR technique respectively. Results: 1. Cx43 eggs were found by immunohistochemical SABC method. White in the two groups of maternal membranes, The positive rate of Cx43 protein expression in fetal membrane epithelial cells in oligohydramnios group was significantly lower than that in control group. The positive rate of Cx43 protein in placental trophoblastic cells was also significantly lower than that in the control group (P < 0.05). The expression of Cx43m RNA was found in the placenta of both groups by RT-PCR method. The expression of Cx43m RNA in the placenta of oligohydramnios group was 0.51 times as much as that of the control group, and the difference was significant (tr 15.792U P0.01P 0.01), and the expression of Cx43m RNA in placenta was 0.54 times higher than that in the control group, compared with that in the control group, the expression of Cx43m RNA in the placenta was 0.54 times higher than that in the control group. The results showed that Cx43 protein and its m RNA were expressed in human fetal membrane and placenta, and mainly in amniotic epithelial cells, choriotrophoblast cells and placental vascular endothelial cells. Placental syncytiotrophoblast .2.The expression of Cx43 protein and m RNA in membranes and placenta of patients with oligohydramnios showed a decreasing trend compared with the normal group. 3. Cx43 in placenta of oligohydramnios and normal amniotic fluid. The change of fetal membrane expression may be involved in the regulation of amniotic fluid volume abnormality, suggesting that Cx43 may play an important role in the absorption pathway of amniotic fluid and the exchange of maternal and fetal fluid. The expression mechanism of Cx43 in fetal membrane is different from that in placenta. In addition to the function of water permeability, it may be related to the function of apoptosis, and the specific function needs further study .4.The Apgar score of neonates with oligohydramnios is lower than that of normal full-term neonates. It is inferred that oligohydramnios may affect the outcome of pregnancy and increase the probability of anoxia and asphyxia in perinatal infants.
