CyclinE及PTEN在上皮性卵巢癌中的表达及临床意义

【摘要】:目的: 检测细胞周期素E(CyclinE)、PTEN在上皮性卵巢癌的表达,分析其与上皮性卵巢癌临床病理因素关系及二者表达的相关性,探究其在肿瘤发生发展中的功用与关联,明了其在上皮性卵巢癌诊断、预后等方面的应用。 方法: 选用链霉素—过氧化物酶(S-P)免疫组化方法。检测CyclinE和PTEN在正常卵巢组织、良性上皮性卵巢肿瘤组织、交界性上皮性卵巢肿瘤组织和上皮性卵巢癌组织中的表达情况。应用SPSS19.0软件包进行统计学分析。采用χ2检验和Fisher确切概率法分析CyclinE和PTEN在各组织中的表达及与临床病理因素之间的关系;采用Spearman相关分析二者的相互关联性。 结果: 1.CyclinE在正常卵巢、卵巢良性肿瘤、卵巢交界性肿瘤、卵巢上皮性癌的表达率分别为5%,14.3%,40.6%,67.5%,呈不断上升趋势,四组之间差异显著(p0.001)。正常组、良性组CyclinE蛋白的表达明显低于恶性组,差别显著(P0.001);交界性组、恶性组之间比较,差异有统计学意义(P0.05);正常组与交界性组、良性组与交界性组之间比较,差别显著(P0.05);但正常组与良性组,差异无统计学意义(P0.05)。 2.在上皮性卵巢癌患者中,高中分化患者肿瘤组织中CyclinE蛋白表达阳性率低于低分化者,,有统计学差异(p0.05);FIGO分期Ⅲ~Ⅳ患者肿瘤组织中CyclinE蛋白表达阳性率高于Ⅰ~Ⅱ期患者,有统计学差异(p0.05);有淋巴结转移的患者肿瘤组织中CyclinE蛋白表达阳性率高于无淋巴结转移患者,差异显著(p0.05)。上皮性卵巢癌CyclinE的表达与年龄、组织学类型均无关。 3.PTEN在正常卵巢、良性卵巢肿瘤、交界性卵巢肿瘤和上皮性卵巢癌中表达率依次为100%,85.70%,81.30%,40%,呈逐步降低趋势,四组之间差异显著(p0.001)。正常组、良性组二者PTEN的表达明显高于恶性组,差别显著(P0.001);交界性组、恶性组之间比较,差异有统计学意义(P0.001);但正常组与良性组、正常组与交界性组、良性组与交界性组之间,无显著差异(P0.05)。 4.在上皮性卵巢癌患者中,高、中分化患者的肿瘤组织中PTEN表达阳性率高于低分化者,有统计学差别(p0.05);FIGO分期Ⅲ~Ⅳ患者肿瘤组织中PTEN表达阳性率比Ⅰ~Ⅱ期患者低,有统计学差别(p0.05);淋巴结有转移组低于无转移组,有显著差异(P0.05)。PTEN的表达与患者的年龄、组织学类型无相关性。 5.在上皮性卵巢癌中,CyclinE、PTEN的表达呈负相关(rs=-0.414,P0.01)。 结论: 1.CyclinE在正常卵巢组织、良性卵巢肿瘤、交界性卵巢肿瘤及上皮性卵巢癌中的表达,呈逐渐上升的趋势。随着FIGO分期的增高,卵巢上皮性癌组织中CyclinE表达上升;随着病理分级的下降,上皮性卵巢癌组织中CyclinE表达上升;有淋巴结转移的患者CyclinE的表达明显升高。结果表明,CyclinE在卵巢上皮性癌的发生和发展中起重要促进作用。 2.PTEN在正常卵巢组织、良性卵巢肿瘤、交界性卵巢肿瘤及上皮性卵巢癌中的表达,呈逐渐下降的趋势。随着FIGO分期的增加,卵巢上皮性癌组织中PTEN表达降低;随着病理分级的下降,上皮性卵巢癌组织中PTEN表达减少;有淋巴结转移的患者PTEN的表达明显下降。结果表明,PTEN在卵巢上皮性癌的发生和发展中起重要抑制作用。 3.CyclinE和PTEN在上皮性卵巢癌中的表达呈负相关。两者之间可能存在协同作用,联合检测对上皮性卵巢癌的诊断、预后有重要的参考价值。
[Abstract]:Purpose :

The expression of cyclin E ( Cyclin E ) and PTEN in epithelial ovarian carcinoma was examined . The correlation between the expression of cyclin E and PTEN in epithelial ovarian carcinoma was analyzed . The utility and association of the expression of cyclin E and PTEN in the development of epithelial ovarian cancer were investigated .

Method :

The expression of Cyclin E and PTEN in normal ovarian tissues , benign epithelial ovarian tumor tissues , borderline epithelial ovarian tumor tissues and epithelial ovarian cancer tissues was investigated by SPSS 10.0 software . The expression of Cyclin E and PTEN in various tissues and their relationship with clinicopathological factors were analyzed by 蠂2 test and Fisher exact probability method .
The correlation between the two correlations was analyzed by using the correlation analysis between the two methods .

Results :

1 . The expression rates of Cyclin E in normal ovary , benign ovarian tumor , borderline ovarian tumor and epithelial ovarian carcinoma were 5 % , 14.3 % , 40.6 % and 67.5 % , respectively .
There was significant difference between borderline group and malignant group ( P0.05 ) .
The difference was significant between the normal group and the borderline group , the benign group and the borderline group ( P0.05 ) .
But there was no significant difference between the normal group and benign group ( P0.05 ) .

2 . In the patients with epithelial ovarian cancer , the positive rate of Cyclin E protein in the tumor tissues of high and high school differentiated patients was lower than that in the low differentiation group ( p < 0.05 ) .
