补肾健脾方联合西药治疗妊娠早期脾肾两虚型先兆流产的临床研究

【摘要】：概述：先兆流产(threatened abortion)是目前临床上较常见的一种妊娠早期疾病之一其临床表现为妊娠早期阴道出血、小腹坠痛、腰酸等。研究目的：本论文通过对先兆流产的中西医治疗现状的概述及相关临床病例的研究,探讨以中西医结合的方式来治疗妊娠早期脾肾两虚型先兆流产,对其进行临床症状改善程度的评价,对其3项血清指标进行对比总结。方法：对符合妊娠早期脾肾两虚型先兆流产诊断标准的100例患者,随机分为试验组与对照组两组,试验组患者口服自拟方补肾健脾方并同时肌肉注射黄体酮注射液和口服地区孕酮片；对照组患者仅肌肉注射黄体酮注射液及口服地屈孕酮片；分别观察并记录治疗各周后其3项指标的检查结果,讨论其结果对两组患者的总体疗效及中医症候学疗效进行分析。研究结果：试验组总有效率98.00%,痊愈4例(8.00%),显效25例(50.00%),有效20例(40.00%),无效1例(2.00%)。对照组50例患者,治疗成功42例,8例治疗期间胚胎停育。对照组总有效率84.00%,痊愈2例(4.00%),显效15例(30.00%),有效25例(50.00%),无效8例(16.00%)。中西药联合用药治疗组临床疗效明显优于单纯西药治疗组(P0.05)。中西药联合治疗组接受治疗后的3项观察指标血清孕酮(P)、血清雌二醇(E2)、人绒毛膜促性腺激素(HCG),较治疗前均有明显的变化。研究结论：采用中西药结合治疗妊娠早期脾肾两虚型先兆流产较单纯西药治疗效果要好；使用补肾健脾方辨治先兆流产对早孕期间临床症状的缓解有较明显的效果；同时能较明显的提高先兆流产患者的各项激素水平,从临床症状方面及实验室检验结果方面均有较明显的疗效。
[Abstract]:Summary: threatened abortion is one of the most common diseases in early pregnancy. Objective: to explore the treatment of threatened abortion with the combination of traditional Chinese and western medicine in early pregnancy by summarizing the current situation of TCM and western medicine treatment of threatened abortion and the study of related clinical cases in order to explore the treatment of threatened abortion with deficiency of both spleen and kidney in early pregnancy. The degree of improvement of clinical symptoms was evaluated and three serum indexes were compared and summarized. Methods: 100 cases of threatened abortion with deficiency of spleen and kidney in early pregnancy were randomly divided into two groups: experimental group and control group. The patients in the test group were treated by oral administration of Bushen Jianpi recipe, intramuscular injection of progesterone and oral progesterone tablets, while patients in the control group were only given intramuscular injection of progesterone and oral diproprogesterone. The results of three indexes were observed and recorded respectively after treatment for each week, and the results were discussed for the analysis of the overall curative effect of the two groups and the curative effect of TCM syndromes. Results: in the test group, the total effective rate was 98.00. The total effective rate was 8.00%, the remarkable effect was 50.00000 in 25 cases, the effective rate was 40.00000 in 20 cases, and 2.00% was ineffective in 1 case. In the control group of 50 cases, 42 cases were successfully treated and 8 cases were aborted during the treatment. In the control group, the total effective rate was 84.00%, 2 cases were cured and 4.00%, 15 cases were significantly effective, 25 cases were effective and 50.00,8 cases were ineffective. The clinical effect of combined Chinese and western medicine treatment group was better than that of pure western medicine treatment group (P 0.05). There were significant changes in serum progesterone, estradiol E _ 2 and HCG in the treatment group after treatment. Conclusion: the combination of traditional Chinese medicine and western medicine in the treatment of threatened abortion with deficiency of both spleen and kidney in early pregnancy is better than that of western medicine alone, and the treatment of threatened abortion with Bushen Jianpi recipe has obvious effect on the relief of clinical symptoms during early pregnancy. At the same time, it can obviously improve the hormone level of threatened abortion patients, and has obvious curative effect from clinical symptoms and laboratory test results.
